On September 30, 2019, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection at James A. FitzPatrick Nuclear Power Plant. On October 17, 2019, the NRC inspectors discussed the results of this inspection with Mr. Joseph Pacher, Site Vice President, and other members of your staff. The results of this inspection are documented in the enclosed report.
The NRC inspectors did not identify any finding or violation of more than minor significance.

FitzPatrick Nuclear Power Plant in accordance with the Reactor Oversight Process. PLANT STATUS
FitzPatrick began the inspection period at rated thermal power, and remained at or near rated thermal power for the remainder of the inspection period.
INSPECTION SCOPES
Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors performed plant status activities described in IMC 2515 Appendix D, Plant Status, and conducted routine reviews using IP 71152, Problem Identification and Resolution. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

71153 - Followup of Events and Notices of Enforcement Discretion Event Report (IP Section 03.02)
The inspectors evaluated the following licensee event reports (LERs):
- (1) LER 05000333/2018-001-00, Secondary Containment Differential Pressure Exceeded the Technical Specification Surveillance Requirement (ADAMS Accession No. ML19032A633). The inspectors reviewed the LER submittal. The inspectors determined that the cause of the condition described in the LER was not reasonably within the licensee's ability to foresee and correct and therefore was not reasonably preventable. No performance deficiency nor violation of NRC requirements was identified. The circumstances surrounding this LER are documented in the Observation table associated with this report. This closes the LER.
Personnel Performance (IP Section 03.03) (4 Samples)
- (1) The inspectors evaluated a tagout on an incorrect component on the auxiliary boiler system and the licensees performance on August 12, 2019.

60855.1 - Operation of an Independent Spent Fuel Storage Installation at Operating Plants Operation of an Independent Spent Fuel Storage Installation at Operating Plants
- (1) The inspectors evaluated FitzPatrick's independent spent fuel storage installation (ISFSI) cask loadings from September 23-26, 2019. Specifically, the inspectors observed or reviewed the following activities:

Heavy load movement of the multipurpose cask and HI-TRAC
Stack up
Canister drying and helium backfill evolutions
Closure welding and non-destructive weld evaluations
Helium leak testing
Radiological field surveys and radiation protection coverage of ISFSI activities
Movement of the HI-TRAC to the ISFSI pad
INSPECTION RESULTS
Observation: IR4200410 - Failure of Instrument Air Compressor 39AC-2A and Failure to Maintain Secondary Containment Differential Pressure Within Technical Specification Limits 71152 On December 5, 2018, FitzPatrick experienced a trip of the service air compressor, 39AC-2A, which caused a loss of instrument air pressure and subsequently caused the reactor building ventilation to isolate. This led to an increase in secondary containment differential pressure to a condition outside of Technical Specifications. This condition existed for approximately 3 minutes before differential pressure was restored to normal when the standby gas treatment system was manually initiated.
The inspectors performed a review of Exelon's corrective action program evaluation (CAPE),associated procedures, condition reports, a work order, and an engineering change report; and conducted interviews with Exelon staff. The inspectors focused on cause identification, extent of cause, and extent of condition to determine if Exelon staff adequately addressed the event and developed appropriate corrective actions to prevent recurrence.
Exelon identified the cause to be the failure of a check valve internal to the 'A' service air compressor. Specifically, the check valve failed and remained in the open position. This allowed system header pressure to backflow through the compressor while it was unloaded and caused it to trip on high intercooler pressure. The licensee was not able to determine the cause for the failed check valve, but in accordance with PI-AA-125,1003, Corrective Action Program Evaluation Manual, Revision 5, Section 4.4.1.5, Exelon performed a risk assessment to determine the risk associated with continued operation in that configuration.
Exelon performed an extent of condition review and identified that the 'B' and 'C' service air compressors were also susceptible to the same failure due to being of the same design.
Based on the vulnerability, Exelon installed an additional check valve in line with the existing check valve to improve reliability and reduce the risk of failure to isolate the instrument air header.
The inspectors determined that Exelon's actions were in accordance with applicable station procedures, and provided reasonable assurance that the risk of losing instrument air header pressure has been reduced. Specifically, the inspectors determined that the risk assessment as well as the addition of a second check valve reduced the probability of recurrence of the event.
