Management Meeting The Director of Region II and members of his staff met with Tennessee Valley Authority Management representatives on November 19, 1980, at the Region II office in Atlanta, GA.
The purpose of this meeting was to discuss test control deficiencies identified during inspections of construction and preoperational testing conducted during the period November 2-14, 1980 at Sequoyah Unit-2.
Discussion focused on the means of assuring that con-struction test procedures are followed and monitored.to ascertain procedural compliance, the review for technical adequacy the-applicable construction tests and test results, and provisions to assure that overall integrated test objectives are met.
TVA representatives were responsive and indicated that action was being taken to correct these deficiencies and would be applicable to other

facilities undergoing construction.
Corrective action would include the following:
a.
Reactor coolant system hydrostatic testing procedures will be prepared by Division of Construction for review and approval by Division of Engineering Design, Division of Nuclear Power, and the site construc-tion QA unit prior to use. Tests will be conducted by a Construction test director with services provided by the Nuclear Power Division.
Test results will be reviewed by all four of the aforementioned organizational units.
Additionally, future system hydrostatic tests
.will have an individual test procedure for each test which has been checked and verified by Construction.
These procedures will specify test details and criteria such as:
pressure, water chemistry, hold points, and test boundary. Each test procedure and its results will be reviewed by Construction QA.
b.
Construction test directors will be required by administrative controls to' ensure that; all test personnel use the procedure; that prior to initiating the test and prior to shift turnovers the test team is
. properly briefed; and that test boundary interfaces and supporting services are coordinated with the Nuclear Power Division shift engineer.

The. construction test program relative to cleaning and flushing is c.
being reinforced similarly to tne hydrostatic testing program with emphasis on test control; procedure compliance; and quality of the testing program.

Results of previous hydrostatic testing on Sequoyah Unit 2 have been reviewed by a special team. and discrepancies are being reviewed and

evaluated for resolution.
Retests will be conducted if necessary.
Sequoyah Unit 1 will be reviewed b: the same team and to the same

criteria as Unit 2. 'Any discrepancies will be properly resolved.

However, representatives stated that the fundamental rudiments of the l
construction test program were sound and met 10 CFR 50, Appendix B

l requirements. - TVA intended to review the overall construction test control program 1to assure that present requirements for tests meet the

requirements of Criterion XI of Appendix B and that the procedures and records are adequate.
