SUBJECT: REPORT NOS. 50-321/86-01 AND 50-366/86-01 Thank you for your response of March 10, 1986, to our Notice of Violation issued on February 12, 1986, concerning activities conducted at your Hatch facility. We have evaluated your response and found that it meets the requirements of 10 CFR 2.201.
We will examine the implementation of your corrective actions during future inspections.
In your response to Violation 1, you indicated that you disagreed with the statement in the Notice of Violation that Violation 1 was similar to one reported i
in Inspection Report No. 50-321/85-15.
You stated that the violation in

Inspection Report No. 50-321/85-15 resulted because the procedure lacked l
sufficient detail for the proper accomplishment of the task, while Violation 1 in

inspection Report No. 50-321/86-01 was for failure to follow procedures.
You also stated that the event reported in Violation 1 was an isolated incident.
It is our position that in both Notices of Violation, you were cited for failure to follow plant procedures involving the operation and calibration of radiation detection instruments.
It should be noted that in your response to the violation reported in Inspection Report No. 50-321/85-15, you stated that the reason for the violation was the failure to follow plant procedure and was the result of personnel error.
The identification of two examples of failure to follow the operation and calibration procedures for radiation detection instruments during the inspection reported in Inspection Report No. 50-321/86-01
,
is not considered by the NRC as an isolated event.
