The inspection was conducted in accordance with IE Inspection Procedure Nos.
30703, 38701, 92701, 92702, and 92720.
Results:
One violation with three examples was identified (Criterion VII-failure to adequately control purchased material-Paragraphs 3b(l), 3b(2),
and 3b(3).
Of the three previous inspection findings reviewed, one was closed.

(Open) Violation (31S/83018-02; 316/83019-02):
The NSDRC failed to review audits and audit results.
The inspector verified that the NSDRC was performing a review of audits and audit results.
Review were thorough and adequate; however, the period required to complete the review and issue the report was excessive.
In dis'cussing the matter with licensee personnel, the inspector noted that present schedules provide for bringing the reviews current by October 1986.
This item will remain open pending further reviews to verify timelines (Closed) Violation (315/85007-05D; 316/85007-05D):
Failure to issue the annual condition report trend and evaluation report.
Procedure PMP-7030, RPT.
003 now requires that this report be issued quarterly.
The inspector reviewed reports which had been prepared for the last quarter of 1985 and the first two quarters of 1986.
No further concerns were identified.

Open Item (315/86005-05):
Methods used to qualify suppliers do not appear to determine their capability to perform required testing.
In reviewing quality assurance records of supplier audits and survei llances, the inspector noted that,'n a surveillance of the Bussman Division of Cooper Industries pertaining to purchase order 05587-821-5X, licensee QA personnel had determined that the supplier did not have test equipment capable of performing the required tests.
Subject, to the surveillance, thi s supplier had previously been determined to be conditionally qualified to supply the product.
Based on the surveillance results, the material was placed on hold by the AEPSC Quality Assurance Department until the required tests were performed.
The inspector has no further questions regarding this matter at this time.
Ins ected Corrective.Action 92720 The inspector reviewed current corrective action practices at the D.
C.
Cook Plant and at the American Electric Power Service Corporation Offices in Columbus, Ohio.
All tracking systems used for tracking corrective action items were reviewed and found to be acceptable.
The following tracking systems were reviewed:

Quality Assurance Internal Tracking Log dated August 8, 1986 (4)
Quality Assurance External Tracking Log dated August 8, 1986 All tracking systems appeared to be functioning properly.
Several items were noted to be past scheduled due dates, but none had been past due for a substantial length of time.
A review of selected past due items indicated that appropriate action was being taken.
The inspector reviewed the Quarterly Condition Report Status Report dated April 4, 1986, and noted that 925 condition reports were open.
Approximately forty percent of these were from prior years.
The inspector selected four of the CRs which had been open for some time and reviewed the action that had been taken.
Actions were adequate in two of the four.
For the other two, one had been misplaced for a substantial period and the other was being held awaiting a reply from a line organization.
Licensee personnel later identified that the line organization had responded more than a year earlier.
The
inspector discussed the need for continuous followup of CRs that remain open for long periods of time.
This item will be reviewed as part of the NRC's follow up to Violations Nos.
315/85007-05C and No. 316/85007-05C.
The inspector reviewed audit records for audit QA-86-20.
It was conducted from June 5, 1986, to August 1, 1986, on corrective action at D.
C.
Cook.
The audit was properly conducted and it was thorough.
Eleven deviations of rather minor significance were identified during the audit.
The inspector also reviewed additional corrective actions which had been taken to address the cause of procedural violations which were noted in NRC Inspection Report No. 85007.
A video tape has now been prepared which emphasizes I & MCO and AEPSC management support for procedural compliance and discusses disciplinary actions when individuals do not comply with procedures.
The latest draft of revi sed procedure PMI-7030 describing the updated corrective action program was reviewed.
This program will include trending, and it is expected to be implemented by October 31, 1986.
This inspection revealed that considerable improvement had been made in the D.
C.
Cook corrective action program since shortcomings were identified in early 1985.
Corrective action, related tracking systems, has been vastly improved with few items past scheduled completion dates.
The number of open condition reports has also been reduced from 2131 in February 1985 to 925 in April 1986.
Reviews of currently issued condition reports were timely.
The quality assurance and quality control organizations are actively involved in the tracking, followup and resolution of quality problems, including required corrective actions.
The trending program and revised corrective action program are presently scheduled for implementation by October 31, 1986.
Procurement Control 38701 The inspector reviewed D.
C.
Cook procurement control practices at AEPSC offices in Columbus, Ohio.
The review included purchasing department practices and the methods used by Quality Assurance to select, evaluate and qualify suppliers.
Purchase requisitions are prepared by all AEP plants and offices and are then sent to AEPSC offices in Columbus for procurement.

Cook, only those requisitions requiring approval by AEPSC and those originated by AEPSC in Columbus are routed to AEPSC Purchasing for procurement.
A Qualified Suppliers List (QSL) is used for selection of suppliers for safety-related (N) procurements.
In addition to suppliers for safety-related items, the QSL also lists qualified suppliers for fire protection items and calibration services.
Quality requirements are added to the purchase order by standard "boilerplate" paragraphs which are selected based on the type of material purchase Activities of the purchasing department were adequately controlled.
Questions regarding material requirements and qualified suppliers were appropriately referred to either engineering or quality assurance.
During the review of the methods used by quality assurance to select, evaluate and qualify suppliers for the QSL, the following observations were made:

The inspector noted that Bussman Division of Cooper Industries was placed on the QSL as conditionally acceptable, even though the results of an AEPSC audit conducted November 8, 1985, indicated that 17 of the 18 Appendix B criteria were deficient.
Based on comments on the evaluation summary sheet, most of the deficiencies appeared to be significant.
This failure to provide appropriate source evaluation and selection is in violation of 10 CFR 50, Appendix B, Criterion VII (315/86031-01A; 316/86031-01A).

Sixty fuse blocks were ordered from Bussman through Consolidated Electric Distributors on purchase order 05587-821-5X.
The purchase order was issued on December 9,
1985.
AEPSC QA placed the material on hold as the result of a surveillance conducted January 21-22, 1986.
The material was received by D.
C.
Cook Stores on February 19, 1986.
There were no records in the file indicating that further action had been taken.
This failure to provide adequate control of purchased material is also in violation of 10 CFR 50, Appendix B, Criterion VII (315/86031-01B; 316/86031-01B).

In reviewing the QSL,,the inspector noted that CDC Valve Company of Kansas City, MO and ENSA, Inc. of Getzville, NY were added to the QSL as conditional suppliers without an audit, survey, past performance history, or other qualifying information.
This failure to provide appropriate source evaluation and selection is also in violation of 10 CFR 50, Appendix B, Criterion VII (315/86031-01C; 316/86031-01C).

Cook on August 15, 1986, and at AEPSC Offices in Columbus, Ohio on August 22, 1986, and summarized the purpose, scope and findings of the inspection.
The inspector discussed the likely informational content of the inspection report with regard to documents or processes reviewed by the inspector during the inspection.
The licensee did not identify any such documents or processes as proprietary.
