. Notice of Violation Failure to Follow Procedure
'
Technical Specifications 6.10 and 6.11 for Arkansas Nuclear One, Units 1 and 2, respectively, require that procedures for personnel radiation safety shall be adhered to for all operations involving personnel radiation safety.
Procedure 1000.031, " Radiation Protection Manual," states in Section 6.7.2.B
"All workers are responsible for following RWP (radiation work permit) requirements during each access to an RCA (radiological controlled area)... Workers shall review and acknowledge understanding of the RWP immediately prior to each RCA entry."
Radiation Work Permit 892249 issued November 27, 1989, to cover work activities associated with minor maintenance activities, requires, "Whenever exiting any radiological controlled area outside of controlled access, all personnel will... proceed immediately to PCM (personnel contamination monitors) and perform a whole body frisk in the PCM-1B."

Contrary to the above, on December 13, 1989, an individual stated to the I
inspector, during an interview, that he worked in the controlled area around the old radioactive waste building on December 10, 1989, but did not perform
,i a whole body frisk after leaving the area.
The licensee acknowledged that the individual had not read RWP 892249, which covered the work.

This is a Severity Level IV violation.

Response to the Violation 313-368/8947-01 l
(1) The reason for the violation:
The reason for the violation is attributed to personnel error.
The indi-vidual involved in this incident admitted that he did not read the Radiation Work Permit (RWP) and, subsequently, failed to perform a whole body frisk in accordance with the requirements of the RWP.
(2) The corrective steps which have been taken and the results achieved:
The individual received counselling and a written reprimand. The individual acknowledged that he understood the severity of his error.
During the daily Manager's Meeting on February 16, 1990, the importance of ensuring that personnel read and understand the information in RWPs was emphasized.

A program is being implemented which will identify personnel failing to
read their RWP.
Personnel observed failing to read their RWP will be restricted from entering RCAs until appropriate corrective action is taken by their management.
The individual's supervision will be responsible for administering disciplinary actions and for notifying the Manager - Rad Protection and Radwaste of the corrective actions taken.
The Manager - Rad Protection and Radwaste will review the corrective actions to determine if the individual's access to the RCAs should be reinstated.
.u A memorandum was distributed to all ANO personnel on February 20, 1990, informing them of the new policy regarding the increased attention to
" failure to read RWP" incidents and enforcement of the requirement.
This incident also pointed out potential weaknesses in the location, quantities, setpoints and proper use of personnel contamination monitors on site.
An evaluation is in progress to address each of these items.
This evaluation will be completed by April 20, 1990.
Additionally, specific training sessions were scheduled to address the operation and proper use of the PM-7 Portal Monitor.
This training was offered during the period from December 19, 1989 through January 31, 1990.
As an interim measure prior to completion of this training, a health physics technician was assigned to the Main and Secondary Guard Stations, 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> per day, to monitor the egress of plant personnel through the PM-7 Portal Monitors as personnel exited the plant.
(4) The date when full compliance will be achieved:
