Noti-ce of' Violation Failure to Follow a Maintenance Procedure Unit 1 Technical Specification 6.8.1 requires that written procedures be established, implemented, and maintained covering the activities

.

recommended in Appendix A of Regulatory Guide 1.33, November 1972.

Section I of this appendix includes a recommendation for procedures for maintenance which can affect performance of safety-related equipment.

Maintenance Procedure 1402.010, " Unit 1 Primary Make-up Pumps (P36 A, B & C) Inspection / Repairs," has been established in accordance with this Technical Specification requirement.

Step 4.4 of Attachment 1 to this procedure requires that the coupling between the gear case and the pump be lubricated with Kop-Flex KHP grease.

Contrary to the above, on May 10, 1989, the NRC inspector found that i

Kop-Flex KSG grease was used instead of KHP grease as required to lubricate the gear case to pump coupling on High Pressure Injection Pump P36A on November 8, 1988.

At that time, Procedure 1402.010 was

,

being used to repair the inboard mechanical seal and the outboard bearing housing shaft seal on P36A under Job Order 741600.

This is a Eeverity Level IV violation.

I Response to Violation 313/8918-01 (1) The reason for the violation if admitted:

l AP&L does admit the violation occurred as stated above.

Based i

upon a review of the condition and discussions with the Maintenance Technician who performed the subject. work, the primary reason for the violation appears to be p,ersonnel inattention to detail.

The motor-to pump couplings for the ANO-1 makeup pumps (P36 A, B

& C) consist of a low speed motor-to gear-case section and a high speed pump-to gear-case section.

Use of Kop-Flex KSG grease is specified for the low speed coupling section and Kop-Flex KHP grease is specified for the high speed coupling section in both the applicable maintenance procedure and preventive maintenance data records for these pumps.

Only the pump-to gear-case section was uncoupled, lubricated (requiring use of KHP grease), and recoupled as part of the job order work activities.

However, the material ticket prepared by Maintenance personnel for the job order incorrectly specified KSG grease for use in reassembly of this section of the pump coupling.

As a result, the incorrect grease was drawn from the storeroom and installed.

The Maintenance Technician who prepared the job order material ticket most likely read the procedure and/or preventive maintenance data records incorrectly.

Additionally, the Maintenance Technician who performed the maintenance procedure failed to notice the incorrect grease was being used (possibly due to the similarities between the KSG grease and KHP grease containers).

l (2) The corrective steps which have been taken and the results achieved:

l The individual who performed the maintenance was reprimanded for failing to adhere to the procedure requirements.

The individual who prepared the material ticket was no longer. employed on-site.

.

l The event was discussed with other Maintenance personnel involved

in the-performance of the job order.

Additionally, a memorandum was issued to Maintenance Department personnel describing the event.

The memorandum pointed out that

it is the performer's responsibility to ensure the correct

lubricant is used in the preper location.

Personnel were j

reminded that disciplinary act,fon could result from actions not

in accordance with procedure requirements.

An evaluation by Engineering had been performed in May 1988 providing acceptability of the use of KSG (standard) grease in i

place of KHP (high speed)' grease in the pump-to gear-case

)

coupling sections for the ANO-1 makeup pumps.

The two greases

-

i are compatible and either provides adequate lubrication.

The use of a high speed grease improves the service life of high speed couplings.

Therefore, there was no concern relating to the operability of the pump.

However, to comply with procedural requirements, the pump-to gear-case coupling was purged, cleaned, and lubricated with KHP grease.

(3) The corrective steps which will be taken to avoid further violations.

The corrective actions taken already (as discussed above) address.the cause of the event and are considered sufficient to prevent recurrence.

(4) The date when full compliance will be achieved:

Full compliance was achieved June 19, 1989, when the puenp coupling I

was lubricated with the correct grease.
