(CLOSED) Unresolved Item 50-302/85-16-02, Pipe Support Installation Discrepancie This matter concerns concrete expansion anchor capacities used -in the calculations compared with the licensee's on site testing. It was noted that some of the calculations utili;ing 7/8" dia., 1 1/8" dia., and 1 1/2" dia. , concrete expansion anchor capacities were interpolated from

,

test data. The licensee was requested to justify interpolated data used l

in calculations.

The -inspector reviewed icorrespondence from the licensee's : A/E firm (Gilbert / Commonwealth, Inc.) dated April 16, 1985, to FPC, providingL af calculation that formally documents the derivation of shear capacities for the anchors in question. _ The initial ~ WEJ-IT ' expansion anchor capacities were -interpolated from Crystal . River 3 test data based upon a manual best

'

fit curve; .The A/E also stated that the development of the samelcurve-usi_ng a regression analysis verified that the interpolated -values- '

established in the project design guides ~ were conservative in all case This matter is considered close .~ Unresolved Items Unresolved items were not identified during this inspectio . (CLOSED) IE Bulletin 79-02, Pipe Support Baseplate Designs Using Concrete Expansion Anchor Bolts (25528). Document Review The inspector reviewed the following documents 'to determine' whether the IE Bulletin 79-02 requirements have been adequately. addressed and implemented. determined that the requested actions of the bulletin have been acceptably addressed. The inspector held discussions with licensee's representatives regarding the implementation of the NRC requirements and the licensee commitments, and reviewed the aforementioned supporting documentation to verify that the actions identified in the response have been completed. This bulletin is considered close . (CLOSED) IE Bulletin 79-14, Seismic Analysis for As-Built Safety-Related Piping Systems (25529). Document Review This inspector reviewed the following documents associated with the Bulletin 7S-14 requirements and the licensee commitments to determine whether they have been adequately addressed and implemented. The review of procedures, specifications, and field inspectors have been documented in previous inspection report Letter FPC to NRC, dated October 30, 1979, enclosing a copy of IEB 79-14 Report for Crystal River 3 prepared by Gilbert Associates, In Letter FPC to NRC, dated November 9,1979, resubmitting revi-sions to three pages of the IEB 79-14 Report forwarded October 30, 197 Letter FPC to NRC, dated July 16, 1980, responding to the open items of IEB 79-14, identified in NRC Inspection Report 50-302/80-2, dated February 21, 198 Parameter Report IE-142, dated December 13, 1985, giving status of closecut for IE Bulletin 79-14, shows area of concern Bulletin Closure This inspector has reviewed the above documentation and the docu-mentation in reference to the closure of IFI 81-24-02, reviewed in paragraph 7.b and determined that the requested actions of the bulletin have been acceptably addresse The inspector held discussions with licensee's representatives regarding the imple-mentation of the NRC requirements and the licensee commitments, and reviewed the aforementioned supporting documentation to verify that the actions identified in the response have been completed. problem with pipe supports MSH-219 and MSH-22 As a result, maintenance inspected the above two supports plus two additional supports and correrted any deficiencies. The inspector reviewed Work Request Forms (WRF) # 30211, dated January 8, 1982 and # 30212, dated January 8,1982, that were issued for the rework of the above hangers and found them to be acceptabl The inspector chose Modification Approval Records MAR 84-10-08-01 to verify that the modifications / repairs for the following supports were completed: MUH-871 R/4, DHH-521 R/3, and SWH-84 R/3. The inspector reviewed sketches, NQE Inspection Plans, and WFR #60571 for the three supports. A field inspection of the three supports was also conducte No discrepancies were found for any of the three supports. This matter is considered close b. (CLOSED) Inspector Followup Item, 50-302/81-24-02, Concrete Expansion Anchor Bolt Procedure Chang This matter concerns the lack of cross referencing and overlapping of two procedures used for the installation and inspection of concrete expansion anchor bolt The licensee implemented the following changes to correct the instructions for concrete expansion anchor bolts. The licensee reviewed all MARS requiring the installation of WEJ-ITS and issued Field Change Request (FCR), as required, to provide the correct installation procedures. To reduce the overlap and confusion of existing Maintenance Procedures (MP), a new revision of MP 301 was issued on January 29,1982 (Rev. 5), which was a complete rewrite to allow for the maintenance and integrity verifi-cation of existing expansion anchors. To reduce confusion, MP 515 was deleted in Rev. 2, dated January 29, 1982. For the installation of Concrete Anchor Bolts manufactured by WEJ-IT, MP-517 delineates all requirements for expansion anchors being used by Crystal River With the implementation of the above changes, the installation of concrete anchor bolts now have the proper installation procedures to ensure compliance to all requirement This item is considered closed.
