Areas Inspected:

Structural Modifications to the Mark 1 Containment System The inspector inspected the below listed areas of mod.ifications of the wet-well torus and associated supports to ascertain compliance with IWA-4000 of the ASME Boiler and Pressure Vessel Code,Section XI with Addenda through Summer of 1978.

Weld Procedure Review

.

The weld procedure specification BE-SMB-TN (SMAW) and the five proce-dure qualification records SM 171, SM 235, SM 258, SM 272, and SM 273 were reviewed for compliance to the regJirements for essen'tial vari-ables required by ASME Section IX, Summer 1978 Addenda. This proce-dure is used for the attachment welding of the saddle support to the torus shell and other welds that utilize the shielded metal arc weld technique. No item of noncompliance were identified.

Observations

.

The inspector observed the work activities in progress and completed work relative to the structural modifications bei.ng performed on the Mark 1 torus supports. The inspector examined the work itens for any obvious defects or noncompliance with regulatory requirements, appli-cable codes and fabrication specification FS-2255-1.

Particular note was taken of the presence of quality control inspectors and quality control evidence such as inspection check sheets, records, and weld surveillance.

During the inspection it was n'oted that the Magnetic Particle Exami-nation (MTE) of the butter / temper layer of weld metal, required by Fabrication Specification FS-2255-1 had been discontinued on the basis that a visual examination was adequate.

The inspector was informed that several of the welds had preYiously been examined using the MTE technique and no defects were observed and this pro-vided the basis for eliminating the examination.

Subsequent dis-cussions by the inspector and the licensee representive were held, and the examinations were resumed. The inspector was informed by telephone that all of the remaining accessible weld had been exa-mined by MTE.

No items of noncompliance were identified.

Welder Qualification

.

The inspector audited the qualification records of randomly selected welders engaged in the welding of the torus and attachments.

The requirements in the welder performance qualifications were listed in specification no. BE-WPQ-1, Rev. 1 and ASME Section IX Summer 1978 Addenda and ASME Section III Summer 1978. The weld procedure used for the qualification and the welder identification are sum-marized below:

Inservice Inspection Activities a.

The inspector reviewed records associated with the inservice inspec-tion activities being performed during the current refueling.

The review was made to ascertain compliance with the ASME B&PV Code,Section XI,1974 Edition and including the Summer 75 Addenda. The following items were reviewed.

Reactor Water Clean-up Welds 12-1-2,12-1-5, and 12-1-4, RCIC

13-1-4 and 13-1-5, Main Steam 1-B-8,1-C-9,1-D-8,1-C-8, and 1-D-9, Feedwater 6-B-11, 6-B-12, 6-B-13, 6-A-11 and 6-A-12.

No items of noncompliance were identified.

The inspector reviewed personnel qualification records for six non-destructive examination personnel who were engaged in inservice activities. The review was made to ascertain compliance with ASME Section XI and SNT-TC-1A.

The inspector perfonned a detailed review of ultrasonic test pro-l cedure, Peabody 22.A.16 Revision 3 to ascertain compliance with ASME Section XI,1974 Edition and Summer 75 Addenda.

The inspector noted the procedure did not specifically cover cali-bration sensitivity on the inside surface of pipe welds. The test-ing contractor revised procedure 22.A.16 Revision 3 and incorporated calibration sensitivity at the inner surface. The revised proce-dure 22.A.16 Revision 4 incorporated the inspector concerns.

No items of noncompliance with regulatory requirements were identified.

Exit Meeting

At the conclusion of the inspection on February 22, 1980, a meeting was held at the' Pilgrim Nuclear Station with representatives of the licersee organization. The attendee's names ar~e asterisked in paragraph 1.

The inspector summarized the results of the inspection as described in this report. The licensee representatives acknowledged the inspector's summarization.
