Exit Interview A meeting was held on November 3, 1978, with Mr. R. B. Starkey, Plant Manager, and staff members denoted in paragraph The scope and findings of the inspection were discusse RII Rpt. No. 50-261/78-26 1-2 Review of Licensee Event Reports (LER's)
The inspector reviewed two LER's for consistency with the requirements of Technical Specifications, Section 6.9. The inspector examined the licensee's analysis of the event, the corrective action taken, and discussed the LER's with licensee representative The following LER's were reviewed: LER 78-21, Loss of MCC-5 The inspector discussed corrective action with a licensee representative and determined that corrective actions had been taken to prevent failure of the transfer device that caused this occurrenc This LER is close LER 78-26, Both Containment Spray Pumps Inoperable The inspector discussed the apparent cause and corrective action with licensee representatives and confirmed the designation of the cause of this occurrenc Since plant procedures had not been revised to prevent recurrence as stated in LER, this LER will remain open pending licensee procedure revisions and subsequent review during a future inspectio The inspector noted that since the cause of this event is related to operation in a
depressurized/refueling type mode, procedural changes should be implemented prior to operation in the cause related conditio.
Review of IE Circulars (IEC's)
The inspector reviewed applicable IEC's with licensee personnel to ascertain that IEC's had been received onsite'and reviewed by cognizant personne Licensee's actions on the following IEC's were reviewed: IEC 78-15, Tilting Disc Check Valves Fail to Close with Gravity in Vertical Position. The inspector determined that the licensee had identified each safety-related check valve installed in a vertical positio Further, the licensee determined that no leakage problems had been identified associated with failure of these valves to properly close; and that such failure, should it occur, would be detected by routine surveillance testin The inspector had no further questions on IE Circular 78-1 IEC 78-16, Limitorque Valve Actuator Failur The inspector determined that the licensee had identified each safety-related limitorque valve actuato Further, the licensee determined that

Standing Orders and Operating Notes No problems were identifie However, the inspector noted minor discrepancies and inconsistencies in some of the above documents which were identified to licensee representatives and discussed at the exit intervie The licensee acknowledged the inspectors comment.
Management Staff Plant Tours The inspector reviewed the policy and records pertaining to plant operations assessment by management personnel on weekends and back shifts. Volume 1, Administrative Instruction, Paragraph 4.1.4. of the Plant Operating Manual requires the plant manager or one of his six management level supervisors to conduct a tour of the plant on a weekend or backshift during his scheduled week long tour as the supervisor on cal The records of these inspections were examined for the period of July 1978 through September 197 No problems were identifie.
Implementation of Radiological Controls A periodic review of the licensee's enforcement history indicated that during a one year period from September 1977 eleven items of noncompliance, associated with radiological controls, were cite The inspector discussed the NRC's concern of the apparent weakness in the
RII Rpt. No. 50-261/78-26 1-4 implementation of radiological controls requirements with the Plant Manager. The Plant Manager acknowledged the inspectors concerns and stated that problems in the radiological control areas had been previous recognized and that actions were being considered to correct identified problem The inspector stated that emphasis would be placed on observation of the licensee's implementation of radiological control requirements during future inspection.
Plant Tour The inspector toured the auxiliary building and control room. During the tour of the auxiliary building particular attention was paid to radiation controls being properly established, monitoring equipment recording as required and general plant housekeepin During the tour of the control room particular attention was paid to control room shift implementation and operating personnel shift relief as required in Volume I Administrative Instructions Paragraphs 4.1.1 and 4.1.14 of the Plant Operating Manua No problems were identified.
