DC Cook Unit 1 & Unit 2 SD-121023-001 Rev. 2, 01/13/14 Status: Y I-N U Area Walk-By Checklist (AWC)

Location (Bldg, Elev, Room/Area): Area 25 - AB2, 633, 123 Instructions for Completing Checklist This checklist may be used to document the results of the Area Walk-By near one or more SWEL items.

The space below each of the following questions may be used to record the results of judgments and findings. Additional space is provided at the end of this checklist for documenting other comments.

1. Does anchorage of equipment in the area appear to be free of Yes potentially adverse seismic conditions (if visible without necessarily opening cabinets)?

Panels welded to embedded plates

2. Does anchorage of equipment in the area appear to be free of Yes significant degraded conditions?
3. Based on a visual inspection from the floor, do the cable/conduit Yes raceways and HVAC ducting appear to be free of potentially adverse seismic conditions (e.g., condition of supports is adequate and fill conditions of cable trays appear to be inside acceptable limits)?
4. Does it appear that the area is free of potentially adverse seismic Yes spatial interactions with other equipment in the area (e.g., ceiling tiles and lighting)?

Fire extinguishers OK - long seismic hooks. Block wall - OK per DWG 12-4029, Wall W-13.

5. Does it appear that the area is free of potentially adverse seismic Yes interactions that could cause flooding or spray in the area?
6. Does it appear that the area is free of potentially adverse seismic Yes interactions that could cause a fire in the area?
7. Does it appear that the area is free of potentially adverse seismic No interactions associated with housekeeping practices, storage of portable equipment, and temporary installations (e.g., scaffolding, lead shieldinq)?

DC Cook Unit 1 & Unit 2 SD-121023-001 Rev. 2, 01/13/14 Status: Y fj ] U Area Walk-By Checklist (AWC)

Location (Bldg, Elev, Room/Area): Area 26 - AB2, 633, 123 Fire extinguishers mounted to side of control panels (seismic hooks, but can be an interaction)- Addressed in AR 2012-11730. Big Emergency cart with 4 chocked wheels - small procedure cart chained off to it (no chocked wheels) and TOO CLOSE TO PANEL - Addressed in AR 2012-11726.

8. Have you looked for and found no other seismic conditions that could Yes adversely affect the safety functions of the equipment in the area?

Between 2-NIS-1 and IV panel there are missing bolts connecting the two panels together, but nuts are there (between 2 panels) - needs investigation- AR 2012-11726 was issued to investigate found conditions.

1. Does anchorage of equipment in the area appear to be free of Yes potentially adverse seismic conditions (if visible without necessarily opening cabinets)?
2. Does anchorage of equipment in the area appear to be free of Yes significant degraded conditions?

Support by back left comer of DG has deficiency tag - NRC also questioned it. Rack supportingpipes for 2-XRV-232 AND 2-XRV-231 has I bolt with minor lack of thread engagement. Support for 2-Q T-134-AB - I bolt not fully engaged - OK'd by engineeringand tagged because it would not cause an unacceptableseismic interaction. AR 2012-11485 issued.

3. Based on a visual inspection from the floor, do the cable/conduit Yes raceways and HVAC ducting appear to be free of potentially adverse seismic conditions (e.g., condition of supports is adequate and fill conditions of cable trays appear to be inside acceptable limits)?
4. Does it appear that the area is free of potentially adverse seismic No spatial interactions with other equipment in the area (e.g., ceiling tiles and lighting)?

Fire extinguishers on short hooks - AR 2012-11669 was issued. Block wall on left side of room - OK per DWG 2-4036, Wall W-4 and Wall W-

5. Ceiling light by right back comer has an open hook - OK because it cannot slip off - opening is at an angle towards the top of hook.
