Corninents(Additional pages may be added as necessary)

Status control stantion has minimal self-weight, a weighted base with a low center of gravity, and are interconnected. Therefore, any potential impacts from the stantion during a seismic event would be insignificant and would not cause damage or imapir the function of the MCC. In addition, chatter is not a concern with breakers. This assesment is in agreement with conclusions drawn in seismic house keeping procedure PMP-5020-RTM-001.

Overhead lights are rod hung and adequately secured.

Overhead cable trays are welded to their trapeze supports.

Hoses hung on hooks on the wall -4 ft from the back of the breaker. Hooks are deep and the vertical acceleration at El. 609' of the Auxiliary Building is not large enough to project the hoses from the hooks so they adversely affect the MCCs.

Aneoaoe I. Is the anchorage configuration verification required (i.e., is the item one YE] NZ of the 50% of SWEL items requiring such verification)?

2. Is the anchorage free of bent, broken, missing or loose hardware?

Lower left bolt appears to be missing. There is weld material present inside the nut however no bolt appears to be behind it.

Bolt possibly broken off-AR 2013-4493 was issued.

3. Is tile anchorage free of corrosion that is more than mild surface oxidation?

5. Is the anchorage configuration consistent with plant documentation?(Note: This question only applies if the item is one of the 50% for which an anchorage configuration verification is required.)
6. Based on the above anchorage evaluations, is the anchorage flee of potentially adverse seismic conditions?

Lower left bolt appears to be missing - AR 2013-4493 was issued (See Question 2 above).

An evalaution was completed under AR 2013-4499. The component was determined to be seismically adequate.

Follow-up walkdown was performed in support of the AR evaluation.
