- 2. Is the anchorage free of bent, broken, missing or loose hardware?
Lower left bolt appears to be missing. There is weld material present inside the nut however no bolt appears to be behind it.
Bolt possibly broken off-AR 2013-4493 was issued.
- 3. Is the anchorage free of corrosion that is more than mild surface oxidation?
- 4. Is the anchorage free of visible cracks in the concrete near the anchors?
- 5. Is tile anchorage configuration consistent with plant documentation?(Note: This question only applies if the item is one of the 50% for which an anchorage configuration verification is required.)
- 6. Based on the above anchorage evaluations, is the anchorage fiee of potentially adverse seismic conditions?
Lower left bolt appears to be missing - AR 2013-4493 was issued (See Question 2 above).
An evalaution was completed under AR 2013-4499. The component was determined to be seismically adequate.
Follow-up walkdown was performed in support of the AR evaluation.

Caulked seal at cabinet upper seams was to be replaced per EC-49577, however the seal was very degraded. Not a seismic concern. AR 2013-14849 was written as a work request to replace the seal.
Upon further investigation subsequent to the walkdown, the SQUG SEWS document (SQUG-2-BCTC-AB) was found which documents and accepts the lack of full thread engagement on the 4 anchor bolts. AR 2013-14848 was written to document that further investigation was needed after leaving the field.
Walkdown was performed on 10/04/2013.
