. r LER 82-016/01T-0 Attachment The Turbine Building Ventilation Monitoring System Noble Gas Monitor was determined inoperable on 9-18-82. The applicable action statement was entered and appropriate samples were taken. However, the Mechanical Vacuum Pump was not taken out of operation. This was in violation of the action statement in effect and was therefore reportable per 6.9.1.8.b.

The MVP was placed in operation at 1650 on 9-17-82. Upon realization of the error on 9/20/82, actions were initiated to remove the MVP from service and the Steam Jet Air Ejectors were placed in service.

The MVP was inadvertently left in operation because the on shift operators did not make note of the sentence in the action statement prohibiting op-eration of the MVP. To prevent recurrence of an event of this nature, the shift personnel have been instructed to read all portions of the action statements and assure that L.C.O. log book is properly and accurately up-dated to reflect plant conditions. Further, the L.C.O. log will be reviewed shiftly to keep abreast of limiting conditions.
