UPDATE REPORT - PREVIOUS REPORT DATE APRIL 22, 1983 ATTACHMENT 'IO LER 83-012/3X-01 NORTHEAST NUCLEAR E'EIGY CmPANY MILISKNE NUCLFAR POWER STATION - UNIT 2 FACILITY OPERATING LICENSE NUMBER DPR-65 DOCKET NO. 50-336 CAUSE DESCRIPTIN AND CURRrLnVE ACTICN:

On March 22, 1983 with the A charging pmp tagged out for maintenance and the B+C charging pumps on line, the drive coupling for the integral oil punp of the C charging punp failed, causing a loss of oil pressure, consequently the pump was shut down.

The drive coupling and the integral oil punp were replaced. The cause of the failure of the drive coupling is unknown.

During the repair of the C charging pump, the A charging punp was returr.ad to service.

On March 26, 1983, with the A charging punp tagged out for maintenance and the B+C charging punps on line, the set screw that holds the oil pump drive coupling in place backed out, allowing the coupling to disengage. At this point oil pressure was lost on the C charging pump and the pump was shut

. down.

The drive coupling was reinstalled and the set screw was tightened and staked to prevent future loosening. The set screws for the drive coupling of the A+B charging punps have been inspected, tightened, and staked. The charging punp overhaul procedure has been changed to stake these set screws when the pump is overhauled.

The charging punps are Gaulin model no. NP18; the coupling and set screw are provided by Gaulin and are listed as iterrs 21 (part No. M0019 - oil punp coupling) and 28 (part No. 820185 - oil pump coupling set screw) on Gaulin drawing D15424, Power End Assembly.
