TheEFWpumpwasimmediately secured,"thetripmoduleswerereset,andEFICandEFWwerereturnedtotheirnormalconfiguration.

Therootcauseofthiseventwaspersonnel error.Theindividuals involvedwerecounseled.

EFICisafourchannelsystemthatmonitorsOnceThroughSteamGenerator (OTSG)levelsandpressures, mainfeedwater pumpstatus,RCPstatus/andEngineering Safeguazds Actuation System[JE)channels3and4inordertoinitiateEEWshouldanactuation setpointbereached.TheEFIClogicisa"oneoutoftwotakentwiceformat.ToactuateeithertrainofEEW,atleasttwoofthefourchannelsmustbeinitiated.

IfonlytwoEFICchannelsareinitiated, itispossibletohaveoneorbothtrainsofEFWactuated, depending uponwhichchannelsareinitiated.

ThemonthlyEFICChannel"B"surveillance testwascommenced atapproximately 0825onSeptember 23.Atapproximately 1030,thetechnicians performing thesurveillance verifiedthatasimulated lowlevelinthe"A"OTSGappropriately initiated the"Trip1"logicinthe"A"EEWTraintripmoduleandthe"Trip2"logicinthe"B"EFWTraintripmodule.Afterresetting thesimulated lowOTSGlevel,theprocedure requiredthattheEEWtripmodulesbereset.Theleadtechnician readtheprocedure stepwhichstated,"ResettheEFWTripModulesinChannelsAandB."Thetechnician performing thesteprepeatedtheinstruction; however,insteadofdepressing theresettoggleswitch,hedepressed the"B"EFWtripmoduletrip1button.Thisactionsatisfied theactuation logicforthe"B"trainofEFWandP-7Astarted.SincetheOTSGswereatnormaloperating levelsandpressure, noEFWflowwasinfected.

Thecontrolboardoperatorimmediately securedP-7A.TheEEWtripmoduleswereproperlyreset,EFICandEEWwerereturnedtotheirnormalconfiguration, andthesurveillance procedure wasexited.Thesurveillance testwasrecommenced at1247andsuccessfully completed at1422onSeptember 23.C.RootCauseTherootcauseofthiseventwasdetermined tobepersonnel error.Thetechnician performing theactionunderstood theprocedure stepreadbytheleadtechnician andrepeatedit;however,hefailedtoself-check beforeperforming theaction.NRCFORH366A(5-92)

theerrormightnothaveoccurred.

D.Corrective ActionsThetechnicians involvedwiththiseventwerecounseled bymanagement priortorecommencing thesurveillance test.Theimportance ofself-checking wasemphasized.

Thiseventandmanagement expectations regarding self-checking werediscussed withtheUnit-1Instrumentation andControlshoppersonnel duringtheirmorningmeetings.

Procedure changeswefeimplemented toclearlyspecifydepressing theresettoggleswitchwhenresetting theEFWtripmodules.TheMaintenance HumanPerformance Committee willdevelopasummaryofthelessonslearnedfromthiseventtobepresented totheMaintenance Departments ofANO-1andANO-2.Thesummaryisexpectedtobecompleted byDecember21,1998.E.SafetySignificance TheEFZCandEFWsystemsperformed asdesignedaftertheinadvertent actuation signalwasinitiated.

However,sincetheOTSGswereatnormaloperating levelsandpressure, noEFWflowwasinitiated andnoplantperturbation resultedfromthisevent.,Therefore, thiscondition isconsidered tobeoflowsafetysignificance.
