Siemens Circuit Breaker Potential Part 21

Reference:

2. Wyie Laboratories Potential Part 21 Notification Letter 45510B-48 dated April 21,1998

Dear Sirs:

Pursuant to the 10 CFR Part 21 requirements, this letter provides the findings, resolutions, and conclusions pertaining to Potential Part 21 notification contained in Wyle's 4/21/98 letter along with Notice of Anomaly (NOA) No. 8.

The equipment was supplied to Entergy Operations Inc. (EOl), Russe!!ville, AR under EOI PO No. 982027. NOA No. 8 included two anomalies that are reprinted and resolved as fo!!ows:

Anomaly No.1: The circuit breaker did not latch closed in the test position with the GE test flag installed. As a result, the MOC did not maintain a change of state condition Resolution No.1: The Siemens vacuum circuit breakers should utilize a Siemens test flag.

However, the Siemens flag had not been provided with the breakers to EOI. Subsequent testing at Siemens using the GE test flag also resulted in failure of the breakers to latch closed. In addition, Siemens and Wyk had not supplied EOl with adequate installation and test procedures that may have prevented this occurrence. Now manuals are in preparation, which will provide satisfactory installation and test procedures.

The Siemens test flag has been t(sted on EOI 5 KV and 7KV breakers at Siemens and proven to operate adequately. The conclusions are that: (1) The util.: ties should not attempt to use the GE test flag with the Siemens replacement vacuum breakers and (2) Siemens/Wyle will provide new procedures for the installation and test of the breakers with the Siemens' test flag.

Anomaly No. 2: The stored energy mechanism did not discharge when the breaker was withdrawn from the cubical. \

Resolution No 2: Provisions for adequate adjustments of the positive interlock switch roller I have been added to the design which resoVes this anomaly.

Neither of the two anomalies described above affect the circuit breakers operwoulty wneo the breake/ is in its connected position, i.e., the position required for the breaker to perform its safety-related function. Therefore, a Part 21 condition does not exist.

@EAKERS IN NON-SAFETY RELATED APPLICATIONS AT SALEM / HOPE CREEK.

lCWEVER, THE SIEMENS TECHNICIAN DID NOT INSTALL THE KIT INTO THE CIRCUIT MEAKER PER THE APPROPRIATE SIEMENS DRAWING. THE IDENTIFIED CIRCUIT MEAKER, ALONG WITH NINE ADDITIONAL CIRCUIT BREAKERS WILL BE SHIPPED FROM MKANSAS TO THE SIEMENS MANUFACTURING FACILITY AT WENDELL, NORTH CAROLINA MERE THEY WILL UNDERGO AN APPROVED EVALUATION PROCEDURE.
