All additional findings and all performance indicators were Green.

Safety Significant Findings One White Finding associated with failure to: wear dosimeters at the highest exposed portion of the body and use an NRC approved method to determine the effective dose for workers wearing dosimeters on several areas of their bodies. Corrective actions completed by the licensee included re-calculating dose to the affected workers, informing the workers of their revised dose, and verifying that no worker's dose exceeded any federal limits. Palisades will receive increased oversight and an additional inspection using Inspection Procedure 95001. 8 IP 95001 Supplemental Inspection The objectives of an IP 95001 inspection are to provide assurance that: the cause(s) of the deficiency are understood the extent of the issue has been identified corrective actions will prevent recurrence These inspections typically involve 1-2 inspectors and require about 40 inspection hours of effort. This inspection is expected to be performed later this year. 9 Safety Conscious Work Environment (SCWE) Safety Conscious Work Environment (SCWE) - an environment where personnel feel free to raise safety concerns without fear of retaliation, intimidation, harassment, or discrimination. All NRC licensees and contractors are expected to establish and maintain a SCWE which contributes to the safe operation of NRC-regulated facilities. 10 Security Department SCWE Inspections In December 2013, the NRC identified a chilled work environment in the Security Department. Licensee developed SCWE Action Plan. In May 2014, the NRC determined that the quality of licensee actions were insufficient to assess and understand the cause of the chilled SCWE. Licensee enhanced the SCWE Action Plan. In December 2014, the NRC completed a SCWE follow-up inspection which determined a chilled work environment no longer existed in the Security Department. 11 Follow-up SCWE Inspection The focused SCWE inspection conducted in December 2014 concluded that although the Palisades Security Department no longer had a chilled work environment; the cultural improvements remained vulnerable to events that might be perceived negatively by the security officers.
