In the fourth quarter of 2011 NRC placed Palisades in Column III
- Degraded Cornerstone Column due to:
An Auxiliary Feed Water pump failed to function -
(low to moderate safety significance)
Service Water pump coupling failure -
(low to moderate safety significance)
Complicated shutdown caused by fault during work on an 10 electric panel -
(substantial safety significance)

Follow-up Supplemental Inspections Inspections
- The objectives of the inspections conducted to f ll ith th Whit fi di l t d t th follow up with the White findings related to the Auxiliary Feedwater Pump and Service Water Pump Coupling failure and Yellow finding Pump Coupling failure and Yellow finding related to the complicated shutdown from the electrical fault are:
- the root causes for the risk-significant issues were understood;
- the extent of condition and extent of cause of the issues were identified; and
- corrective actions were or will be sufficient to address and preclude repetition of the root cause.
12
Supplemental Inspections pp p
- Included an independent NRC review of the extent p
of condition and extent of cause.
- Concluded there were no significant weaknesses.
- Independently assessed whether any safety culture
- Independently assessed whether any safety culture component caused or significantly contributed to the performance issues.
- Determined licensees corrective actions were d

t adequate to prevent recurrence.
13
NRC Safety Culture Review y
120 plant employees/contractors were interviewed by the NRC safety culture experts.
The inspection team determined that, although there are still some challenges the safety culture was adequate and is some challenges, the safety culture was adequate and is improving.
Employees at all levels of the organization stated they felt comfortable raising nuclear safety concerns through multiple avenues.

In addition to routine inspections, the NRC will also:
- Ensure that the licensee is implementing appropriate corrective actions to improve the organization and sustain performance on site.
- Ensure that planned corrective and maintenance actions on a number of recent technical issues for which initial NRC review identified no immediate safety concerns, are effective such y
that these issues will not lead to more significant safety issues.
