Facility Form Policy No. NF -0263: ANI will amend the named insured to "EntergyLouisiana, LLC and Entergy Operations, Inc." in lieu of "Entergy Louisiana, LLC andEntergy Operations, Inc." Even though it is not necessary to change the named insuredfor this policy because the names are exactly the same, we have elected to do so inorder to provide documentation to you that we are willing to write nuclear liabilityinsurance for the newly formed Entergy Louisiana, LLC.b. Master Worker Certificate No. NW -0623: ANI will amend the named insured to"Entergy Louisiana, LLC and Entergy Operations, Inc." in lieu of "Entergy Louisiana, LLCand Entergy Operations, Inc." Even though it is not necessary to change the namedinsured for this certificate because the names are exactly the same, again we haveelected to do so in order to provide documentation to you that we are willing to writenuclear liability insurance for the newly formed Entergy Louisiana, LLC.c. SFP Certificate No. N -0100: ANI will amend the named insured to reflect "EntergyLouisiana, LLC and Entergy Operations, Inc." in lieu of "Entergy Gulf States Louisiana, LLCand Entergy Operations, Inc."d. SFP Certificate No. N -0100: ANI will add "Entergy Louisiana, LLC" to the Bond forpayment of retrospective premiums. Entergy Louisiana, LLC is assuming all past andfuture liabilities of Entergy Gulf States Louisiana, LLC for the payment of retrospectivepremiums.The commitment to write insurance for the Waterford and River Bend nuclear power stationsremains valid thru December 31, 2015.
