1 APPENDIX A NOTICE OF VIOLATION Northeast Nuclear Energy Company Docket No. 245 Millstone Point Nuclear Power Station, Unit 1 License No. DPR-21 j I As a result of the inspecticn conducted on July 6-10, 1987, and in accordance l

with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (Enforcement Policy) (1986), the following violations were identified:

l A. 10 CFR 20.203(f)(1) and (2), requires that each <.< ntainer of specified l amounts of licensed material bear a durable, clearly visible label bearing l the radiation caution syn'bol and the words " Caution" or " Danger, Radio-l active Material", and identifying the radioactive contents.

Contrary to the above, on July 6, 1987, a shipping box containing radio-active material and located in the railway access area on the 14'6" elevation of the Unit I reactor building was not labeled as required.

This is a Severity Level V violation. (Supplement IV)

8. Technical Specification 6.11 requires that procedures for radiation protection be prepared, approved, maintained and adhered to for all operaticns involving personnel radiation exposure. Procedure SHP 4912, Rev. 8, Section 4 requires, in part, that workers be responsible for

. reading, understanding, initialing and following RWP instructions."

Section 8 of procedure SHP 4912 also requires, in part, that " ..no further work shall be performed under an RWP once it is terminated or expired. .."

1. Contrary to the above, during the week of July 6,1987, multiple workers initialed and worked on Radiation Work Permit (RWP) #4348, without reviewing the latest Health Physics survey, as required by the RWP.
2. Also contrary to the above, on July 7,1987, a worker initialed and prepared to enter the Unit 1 drywell on RWP #4631 in violation of RWP requirements. Specifically, the worker was not wearing or did not have in his possession respiratory protection equipment as required on the RWP.  ;

3. Also contrary to the above, on July 8, 1987, several workers reviewed, initialed, and performed work under an expired RWP.

Specifically, RWP #4786 listed an expiration date and time of 0FFICIAL RECORD COPY CIR MILL 87-15/17/14 - 0003.0.0 8708210431 870819 PDR 0 ADDCK 05000245 PDR

4 July 8, 1987 at 0630. Six individuals initialed and made drywell entries on RWP #4786 on July 8, 1987 at times subsequent to the 1 expiration time of 0630. J This is a Severity Level V violation. (Supplement IV)

Pursuant to the provisions of 10 CFR 2.201, Northeast Nuclear Energy Company is hereby required t'o submit to this office within thirty days of the date of (

the letter which transmitted this Notice, a written statement or explanation in I reply, including: (1) the corrective steps which have been taken and the l re;ults achieved; (2) corrective steps which will be taken to avoid further  !

violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
