ENCLOSURE NOTICE OF VIOLATION Northeast Nuclear Energy Company Docket Nos. 50-245, 336, 432
- Millstone Station License Nos. DPR-21, DPR-65, NPF-49 During an NRC inspection conducted from May 12 to 14,1992, a violation of NRC requirements was identified.
In.accordance with the " General Statement of Policy and
' Procedures for NRC Enforcement Actions," the violation is listed below:
10 CFR 50.9(a) requires that information required by statute or by the Commission's regulations shall be complete and accurate in all material respects.
Technical Specification 6.8.1.a for Millstone Units 1, 2, and 3 states, in part, that written procedures shall be implemented covering the applicable procedures recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978.
Regulatory Guide 1.33, Revision 2, February 1978, Appendix A, identifies safety-related activitics that should be covered by written procedures.
Item 1.h. under Administrative Procedures identifies log entries as an activity that should be covered by procedures.
Millstone Unit 1 Operations Department Instruction 1-OPS-1.09, " Conduct of Operations," (Rev. 3) Paragraph 15 states, in part: "Each operator shall conduct a thorough inspection of all areas within his responsibility, making appropriate equipment inspections at least once each shift."
' Millstone Unit 2 Station Procedure SP 2669A, " Conduct of Operations" (Rev. 9),
Paragraphs 1 and 2, require, the PEO to complete at least one complete set of rounds per
. shift as specified in the log sheets, and if a second set of rounds cannot be completed, then provide a reason in the remarks section of the logs.
Millstone Unit 3 Station Procedure SP 3670.3, " Control Room and Plant Equipment Rounds," Paragraph 7.1 states, in part: "At least one complete set of rounds per shift for the areas specified on OPS Form 3670.3-3 will be made."
Contrary to the above, the licensee determined that records required to be maintained by Commission regulations or license conditions were not complete and accurate in all material respects.
Specifically, during the period dating from October 1,1991 to May 14,1992, the licensee identified 25 operators, eight of whom were licensed Reactor Operators, who had occasionally or chronically falsified logs indicating they had completed their rounds in areas listed on the log sheets, when in fact those rounds had not been fully completed.
