Notice is hereby provided that your Quality Assurance (QA)

Program Approval for Radioactive Material Packages, Approval No. 0643 expired on December 31, 1992.

If you are an NRC licensee and conduct activities under.the General Licenses of Subpart C of_10 CFR Part 71, or an Agreement l

State Licensee subject to-the requirements of 10 CFR Part-71, as required by 10-CFR S 150.20, a QA Program approved by the Commission as satisfying the provisions of Subpart~H of_10 CFR1 Part 71 is required. Since you no longer have a_ valid QA, Program Approval, the General Licenses of Subpart C of 10 CFR-Part'71.and registration under the provisions of 10 CFR S-71.12, to trans-port or deliver licensed material to a carrier-for transport is-terminated. This. includes your registration as a user of certificate of Compliance Nos. 6717, 9127, 9128,'and.9156.

However, we have registered you for the use'of these packages under the provisions of 49 CFR 173.471.

Please note that records required by 10 CFR S 71.91 are to be'-

retained for a period of three years.
