As requested by your application dated November 6,1995, as supplemented on August 19.-1996 and May 21, 1997, enclosed is Certificate of Compliance No.
9128, Revision No. 7, for the Model No. C 8 asckage. This certificate supersedes, in its entirety, Certificate of Compliance No. 9128, Revision No. 6, dated February 12, 1991.
Changes made to the enclosed certificate are indicated by vertical lines in the margin.
Those on the attached list have been registered as users of this package under the general license provisions to 10 CFR 571.12 or.49 CFR S173.471.
This approval constitutes authority to use this Jackage for shipment of radioactive material and for the package to be slipped in accordance with the provisions of 49 CFR S173.471.
