Notice is hereby provided that your Quality Assurance (QA)

Program Approval for Radioactive Material Packages, Approval No. 0646 expired on December 31, 1992.

If you are an NRC licensee and conduct activities under the General Licenses of Subpart C of 10 CFR Part 71, or an Agreement State Licensee subject to the-requirements.of 10 CFR.Part,71, as' required by 10 CFR S 150.20, a QA Program. approved by the Commission as satisfying the provisions of Subpart H of:10 CFR Part 71 is required.

Since you'no longer have a valid QA Program Approval, the General Licenses of Subpart C of 10 CFR Part 71-and registration under the provisions of 10 CFR S 71.12, to_trans-port or deliver licensed material to a carrier for transport iu terminated.

This includes your registration as'a user of Certificate of-Compliance Nos. 6717, 9021, 9028, 9032, 9033, 9036, 9056, 9107, 9127, 9128, 9148,.and 9156.

However, we.have registered you for the use of these packages:under the provisions-of 49 CFR 173.471.

Please note that records required by 10 CFR S 71.91 are-to be retained for a period of three years.
