PROPOSED IMPOSITION OF CIVIL PENALTY - $100,000 This is to inform the.Comission that a Notice of Violation and Proposed Imposition of Civil Penalty in the amount of One Hundred Thousand Dollars
($100,000) will be issued on or about March 17, 1988 to Wolf Creek Nuclear Operating Corporation. This action is based on several examples of significant procedural control weaknesses during the performance of outage activities.
These weaknesses resulted in, among other things, improper maintenance on safety-related switchgear, contamination of personnel, release of potentially contaminated materials to a landfill, ignition of hydrogen in the pressurizer, an engineered safeguards actuation causing lake water to enter the steam generators, and the movement of fuel when required monitors were inoperable.
The base civil penalty was increased by 100 percent because of the number of examples of violations and because of the failure to take prompt-corrective actions.
It should be noted that the licensee has not been specifically informed of the enforcement action. The schedule of issuance and notification is:
