. Event Description After completing repairs on TV-BD100 A, B, D and F, stroke times were not measured prior to return to service as required by T.S. 4.6.3.1.1.b. Quality Control review of the completed Maintenance Report determined that the valve had been functionally tested, but not timed.

Probable Ccnsequences of Occurrence Each valve had redundant valves to perform the intended function. As a -

result, the consequences were minimal and the health and safety of the public was not endangered.

Cause of Occurrence Personnel involved did not measure stroke time while performing functional tests after maintenance.

Immediate Corrective Action TV-BD-100 A, B, D and F were tested. All stroke times were found to be within specification.

Scheduled Corrective Action A listing of all valves requiring stroke time measurement following maintenance will be added to valve maintenance procedures.

Action Taken to Prevent Recurrence Involved personnel were reinstructed in the requirements of T.S. 4.6.3.1.1.b in regards to the stroke time measuring following maintenance.
