Page 1 of 1 Docket Number 50-338 Report No. LER 78-115/03L-0 Description of Event On November 7,1978, with the unit in normal operation (95.2% power), Periodic Test 1-PT-134 was performed as scheduled. During the test, which requires the chemical addition tank discharge valves to be cycled, M0V-QS102A opened but would not return to its normal closed position. Technical Specification Statement 3.6.2.3 requires that all valves in the chemical addition flow path be in its correct posi-tion. Not being able to meet this technical specification, an emergency maintenance report was submitted to repair Chemical Addition Tank Discharge Valve MOV-QS102A.

Probable Consequences of Occurren'ce The public health and safety were not endangered by the inability of M0V-QS102A to close since the valve must be opened when this system actuates.

Causes of Occurrence The reason for M0V-QS102A not closing was the direct result of dirty contacts on the valve's torque switch. The dirty contacts allowed the valve to remain open when a signal to close the valve was received.

Immediate Corrective Action Once it was determined that MOV-QS102A was unable to close, an emergency maintenance report was submitted to the electrical shop. The work that was performed under the maintenance report was to clean the torque switch contacts on M0V-QS102A per procedure EMP-C-LS-1. M0V-QS102A was retested with satisfactory results.

Scheduled Corrective Action None required.

Actions Taken to Prevent Recurrence This appears to be an isolated event, therefore no further action is required.
