- A LOCA has occurred on Unit 1.

- Both trains of RHR are in service and are aligned to Sump B.

- Low head recirculation is being monitored per F3-17.2, Long Term Core Cooling.

- The following parameters are noted in the Control Room:

The above information indicates ____________________ blockage.

A. 11 RHR Heat Exchanger B. 12 RHR Heat Exchanger C. Sump B Strainer D. Fuel Assembly 3-PEO Justifications:

a. Incorrect. Plausible as F3-17.2 Table 2 lists RHR HX Blockage as potential flow blockage. Also plausible if examinee incorrectly believes 11 RHR HX is blocked and 12 RHR HX is not blocked because 11 RHR disch pressure is higher than 12 RHR disch pressure. This choice is NOT correct because the RHR discharge pressure would increase instead of decrease for RHR HX blockage.
b. Incorrect. Plausible as F3-17.2 Table 2 lists RHR HX Blockage as potential flow blockage. Also plausible if examinee incorrectly believes 12 RHR HX is blocked and 11 RHR HX is not blocked because 12 RHR flow is lower than 11 RHR flow. This choice is NOT correct because the RHR discharge pressure would increase instead of decrease for RHR HX blockage.
c. Correct. Since RHR pump motor current, RHR Pump Suction pressure, RHR Pump Discharge are all decreasing, this indicates Sump B Strainer Blockage per F3-17.2 Table 2.
d. Incorrect. Plausible as F3-17.2 Table 2 lists Fuel Assembly Blockage as a potential flow blockage.

Also plausible as CETCs are increasing; however, in this case RHR flows and pressures are not STABLE; therefore there is no blockage of the fuel assembly.

There is high activity in the ____________ Coolant System due to a ___________________________.

A. Primary Steam Generator Tube Rupture B. Primary Fuel Element Failure C. Secondary Steam Generator Tube Rupture D. Secondary Fuel Element Failure 2-DR Justifications:

a. Incorrect. Plausible as there is high activity in the primary; however, it is not due to a SGTR.
b. Correct.
c. Incorrect. Plausible as secondary coolant would be high for a SGTR; however, in this case the ERCS indications given show high primary activity. Some ERCS Rad level alarm setpoints vary with the actual rad monitor itself; therefore it is plausible that a radiation level for a ERCS point is high but still green in color. Therefore, the examinee still needs to be familiar with approximately what level is high or normal for any rad monitor.
d. Incorrect. Plausible as a fuel element failure has occurred; however, in this case it would not cause high secondary activity.

- A Unit 1 Containment Isolation signal is present.

What additional signal will cause a DIRECT automatic isolation of Instrument Air to Containment?

A. RCS Low Tavg B. Safety Injection C. Containment Spray Actuation D. High-High Containment Pressure (>17 psig) 1-I Justifications:

a. Incorrect. Plausible as a RCS Low Tavg signal is an input to a Main Steam Isolation signal which would cause a isolation of instrument air to containment; however, there would also need to be a high steam flow signal and SI signal present.
b. Incorrect. Plausible as a SI signal is an input to a Main Steam Isolation signal which would cause a isolation of instrument air to containment; however, there would also need to be a high steam flow signal present.
c. Incorrect. Plausible as a containment pressure greater than 23 psig will actuate Containment Spray; however, this signal is NOT an input to isolate instrument air to containment. This is a common misconception.
d. Correct. A containment pressure greater than 17 psig will cause a Main Steam Line Isolation signal.

Since a Containment Isolation signal is already present, instrument air to containment would isolate.
