Palo Verde Unit 1 Technical Specification (Tech Spec) 4.0.5 specifies the Surveillance Requirements for inservice inspection and testing of ASME Code Class 1, 2, and 3 components. This inspection and testing shall be performed in accordance with Section XI of the ASME Boiler and Pressure Vessel Code.

- 2. In all three cases, the pump test results met acceptance criteria at the test time and all were declared operable by the Shift Supervisor.
The routing procedure, then in effect for completed tests was: 1) From Control Room to Drawing and Document Control (DDC), 2) DDC to the Surveillance Program Control Group (SPCG) and, 3) SPCG to the reviewing engineer.
The following corrective actions are being implemented to prevent a recurrence of these events.
- 1. The controlling procedure for the Surveillance Program (73AC-9ZZ04) has been changed to direct DDC to forward all completed pump surveillance procedures directly to the reviewing engineers and then to SPCG.
- 2. The reviewing engineers will be placed on the distribution list for all surveillance test schedules to keep them apprised of pump testing in advance of procedure performance.
- 3. Additionally, the Shift Technical Advisors (STAs) (Utility licensed and non-licensed) perform a review of the completed tests in the control room to fulfill the 96 hour0.00111 days <br />0.0267 hours <br />1.587302e-4 weeks <br />3.6528e-5 months <br /> review requirement. The STAs are onshift

On July 3, 1985, Palo Verde Unit 1 was in Mode 1 and at 38 percent reactor power.
Palo Verde Unit 1 Technical Specification (Tech Spec) 4.0.5 specifies the Surveillance Requirements for inservice inspection and testing of ASME Code Class 1, 2, and 3 components. This inspection and testing shall be performed in accordance with Section XI of the ASME Boiler and Pressure Vessel Code.
Procedure 41ST-1SI10 was performed on the train "B" High Pressure Safety Injection Pump (HPSI)(BQ) on June 22, 1985. The pump test results were not reviewed within the required time limit of 96 hours0.00111 days <br />0.0267 hours <br />1.587302e-4 weeks <br />3.6528e-5 months <br />.
The cause of this is the same as in LER 85-035-00. Procedural errors in the routing of test data caused the review delay. Steps had been taken to eliminate these procedural errors but were not fully implemented at the time of this event. These steps 'are now complete.

The results of the pump performance test that took place'n July 3, 1985 vere not revieved by the reviewing engineers (non-licensed utility and contract) within 96 hours0.00111 days <br />0.0267 hours <br />1.587302e-4 weeks <br />3.6528e-5 months <br /> required by IVP-3220 of the ASME Boiler and Pressure Vessel Code,Section XI. This was discovered by the engineers doing the review.
This event occurred while the unit was in Mode 1 and at 38 percent reactor power with normal operating temperature and pressure.
HPSI "B" pump had been tested on June 22, 1985 at 1107. The required ASME review of the test results vere not completed until July 3, 1985 at 1212. This event is identical vith those incidents identified in LER 85-035-00. At that time the following corrective actions were specified to correct the problem.
- 1. The controlling procedure for the Surveillance Program (73AC-9ZZ04) has been changed to direct Drawing and Document Control (DDC) to forward all completed pump surveillance procedures directly to the reviewing engineers and then to the Surveillance Program Control Group (SPCG).
- 2. The revieving engineers have been placed on the distribution list for all su'rveillance test schedules to keep them apprised of pump testing in advance of procedure performance.
3, Additionally, the Shift Technical Advisors (STAs) (Utility licensed and non-licensed) perform a review of the completed tests in the control room to fulfill the 96 hour0.00111 days <br />0.0267 hours <br />1.587302e-4 weeks <br />3.6528e-5 months <br /> review, requirement. The STAs are onshift 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> a day 7 days a week, so the tests can be reviewed shortly after completion.
The original corrective action taken for this event involved the use of a label to identify ASME Section XI tests. However, the STAs now perform the test review in order to minimize the time between completion of testing and the review of the test data and to reduce the possibility of misplaced test packages, At the time of this most recent event, the corrective action had not been fully implemented. This was determined to be the root cause of the event.
In addition to the corrective action stated, the reviewing engineers will monitor the required surveillance test schedules and will alert DDC on the timeliness of review/requirements.
The ASME review was completed on July 3, 1985. The results of the review revealed that the HPSI "B" pump passed satisfactorily. Therefore this event had no impact on the health and safety of the public.
A similar event was reported in LKR 528-85-035.
