During the recovery phase of a safety injection, the pressurizer heatup rate 0

exceeded the maximum rate of-100 F in any one hour period. The temperature changed at a rate of 151"F in 30 minutes.

Conditiens Prior to Occurrence:

Unit . in Mode 4 prior to initial criticality.

Actions specified in the Technical Specification Surveillance Requirements met due to. inoperable equipment.

Apparent Cause of Occurrence The safety injection caused an insurge it.to the pressurizer causing the temperature in the lower portion of the pressurizer to approach the temperature of the reactor coolant. During the recovery the icvel in the pressurizer was restored to normal level causing the cooler water to be displaced cut of the pressurizer. This resulted in the temperature increan-ing at a rate and by an amount in excess of the Technical Specification limit.

Analynia of Occurrence There was no danger to the health and safety of the public.

Corrective Acticn The temperaturc was restored to within the limits required by Tect.nical Specification 3.4.9.2 Action within the required 30 minutes.
