Tennessee Valley Authority Sequoyah Nuclear Plant LER SUPPLEMENTAL INFORMATION SQRO-50-327/8005 Technical Specification Involved 3.9.2 Reported Undar Technical Specification 6.9.1.13.b Date of Occurrence: 3/11/80 Time of Occurrence: 1220 CST Unit 1 Identification and Description of Occurrence:

Alarm for loss of detector voltage of NIS System actuated noting that source range channel N31 was not operable.

Conditions Prior to Occurrence:

Unit I was in Mode 6 but core alterations were not in progress.

Action specified in the Technical Specification Surveillance Requirements met due to inoperable equipment. Describe.

Confirmed that core alterations were not in progress and restored the channel to service. ,

Apparent Cause of Occurrence:

Fuses L19 and L20 source range voltage cut out relay had been removed thus causing it to be in the cut out state. When the reactor solid state protec-tion system was placed in the " input inhibit" mode for surveillance testing voltage was lost to the detector.

Analysis of Occurrence:

There was no hazard to plant equipment or personnel health or safety. '

Corrective Action:

Replaced the fuses.
