Enclosed you will find the printed paperwork required upon completing the site administered Initial Written Examination by NUREG 1021, Operator Licensing Examination Standards for Power Reactors, Revision 9, supplement 1.

Please NOTE that question #98 should be treated as classified, proprietary, and sensitive or otherwise protected information and should be withheld from public disclosure.

The Form ES-201 -3, Examination Security Agreement, has been completed; two individuals were not capable of physically signing the ES-201 -3 Form, however a telephone interview was conducted where both individuals attested that the conditions of the agreement were intact.

Finally, a third individual could not sign the original ES-201 -3 Form but instead signed a facsimile and his signature is captured on a separate ES-201-3 Form. Post examination comments for potential key changes are also included.

Enclosed are the following:

• ES-201-3, Examination Security Agreement.

• Sixteen (16) original exam answer sheets: 10 SRO & 6 RO for electronic scanning in LXR software (SCANTRON sheets) and copies of the same.

• Sixteen (16) original signed exam cover sheets: 10 SRO & 6 RO and copies of the same.

• A copy of each applicants LXR generated profile report attached to his answer sheet.

• Preliminary grades (RO, SRO-only, SRO-OVERALL scores).

• Statistics on each question for each of portion of the RO and SRO exams.

• Exams as administered (one 75 question RO exam and one 100 question SRO exam).

• Seating chart utilized during the written exam.

• Exam key for both RO/SRO exams.

• NMP-TR-423-F1 5 which list the questions asked by the applicants during the exam with the responses given.

• A SCANTRON form Marked as KEY; LXR generated exam key and an LXR TEST KEY report.

• Compilation of Comments collected from the applicants from the post-exam review with facility response/action stated. This document also identifies the facility reviewed HIGH Miss questions.

• Re-Grade proposal for SRO-ONLY question #77.
