Atapproximately 1349EDST,the"B"SIpumpwasstartedforthemonthlytest.Whenthemotorwasstarted,afewsparkswerenotedcomingfromthemotor.(Asthemotorcameuptospeed,thesparksstopped.)

ControlRoomoperators wereimmediately notifiedofthesparkingcondition, andpromptlydeenergized the"B"SIpumpmotor.The"B"SIpumpwasthendeclaredinoperable.

Thedeclaration ofinoperability placedtheplantintoaTechnical Specification (TS)LimitingCondition forOperation (LCO)forTS3.3.1.4.Theactionstatement ofTS3.3.1.4allowsoneSIpumptobeinoperable forupto72hours.Ifthepumpisnotoperableafter72hours,thereactorshallbeplacedinhotshutdownwithinthefollowing 6hoursandbelowareactorcoolantsystem(RCS)temperature of350degreesFwithinanadditional 6hours.Immediate corrective maintenance wasinitiated.

The"B"SIpumpmotorwasdisassembled andinspected.

Initialvisualinspection byplantandengineering personnel identified thatseveralrotorbarsshowedcrackindications, andonebarappearedtobefullycrackedandbroken.Therotorwaspackagedandshippedoffsiteforrepairs.XI.DESCRIPTION OFEVENTA.DATESANDAPPROXIMATE TIMESOFMAJOROCCURRENCES:

Detailedinspections performed attheoffsitefacilityconfirmed thatone(1)barwasbroken,andtwo(2)otherbarswerecracked.(Thisbrokenbarwasthecauseofthesparkingobservedduringmotorstart.)Repairswerenotabletobecompleted withinthenextseveralhoursonJune13,norwasaqualified sparerotorlocatedbythistime.TheAllowable OutageTime(AOT)ofTS3.3.1.4requiredthereactortobeinhotshutdownbefore1949EDSTonJune13,1994,andbelowaRCStemperature of350degreesFbefore0149EDSTonJune14.Plantmanagement directedthatthereactorbeshutdownandcooleddownwithintheAOToftheTSLCO.OnJune13,1994,atapproximately 1115EDST,withthereactoratapproximately 97sosteadystatereactorpower,theShiftSupervisor declaredanUnusualEvent,asrequiredbytheGinnaStationNuclearEmergency ResponsePlanandprocedure EPIP-1.0, "GinnaStationEventEvaluation andClassification."

Thiswasbasedoninitiating condition "LossofEngineered SafetyFeatures",

Theneedforthereactortobeinhotshutdownbefore1949EDSTonJune13,1994,wasrecognized onJune10.OnJune13,itwasknownthatrepairswouldnotbecompleted withintheAOTofTS3.3.1.4.Thiswasthebasisforinitiating areactorshutdownatapproximately 1115EDSTonJune13.Thecompletion oftheshutdownatapproximately 1614EDST,requiredbyTS,isreportable totheNRC.NRCFORM366A(5-92)

IV.ANALYSISOFEVENTThiseventisreportable inaccordance with10CFR50.73,LicenseeEventReportSystem,item(a)(2)(i)(A),whichrequiresareportof,"Thecompletion ofanynuclearplantshutdownrequiredbytheplant'sTechnical Specifications."

Anassessment wasperformed considering boththesafetyconsequences andimplications ofthisevent.withthefollowing resultsandconclusions:

oTherewerenooperational orsafetyconsequences orimplications attributed tothe"B"SIpumpmotorrotorsparkingbecausepumpmotorfailuredidnotoccur.Znaddition, thesparkingdidnotcontinueafterthestartofthepump.The"B"SIpumpmotorwasimmediately deenergized andthepumpwasdeclaredinoperable.

oTheplantcontinued tooperateinaccordance withtheLCOofTS3.3.1.4forthenext72hours.Thecrackedrotorbarwasnotrepairable withinthis72hourAOT,sothereactorwasplacedinhotshutdownwithinthefollowing 6hoursandbelowaRCStemperature of350degreesFwithinanadditional 6hoursperTS.Thereactorwasoperatedwithintherequirements oftheTSatalltimes.HRCFORM366A(5-92)

Program,priortothisevent.MotorsinuseatGinna,oftheframesizesofconcernintheWTB,wereidentified andlistedatthetimeoftheassessment.

ThislistofGinnamotorswasreferredto,andvibration analysiswasconducted forselectedmotors.Resultsofthisanalysisindicated that,the"C"SIpumphadpossiblecrackedrotorbars.The"C"SIpumpmotorwasdisassembled andvisuallyinspected.

Novisualevidenceofrotorbarcrackingwasidentified.

The"C"SIpumpmotorwasreassembled, tested,andreturnedtoservice.B.ACTIONTAKENORPLANNEDTOPREVENTRECURRENCE:

ThePredictive Maintenance Programisbeingenhancedtouseimproveddiagnostic vibration analysismethods.Diagnostic testingwillbeperformed periodically totrendformotorinconsistencies, including rotorbarcracking.
