Reported Under Technical Specification 6.9.1.13.b Date of Occurrence: 4/15/80 Identification and Description of Occurrence:

All charging pumps were taken out of service so that maintenance could 1

be performed on valve 62-526. The valve was leaking and needed repacking.

The system design is such that the valva cannot be isolated if either centrifugal charging pump is operating.

Conditions Prior to Occurrence:

Mode 5 Apparent Cause of Occurrence:

i Old or defective packing resulted in valve packing leak when the valve was first operated.

Analysis of Occurrence:

None Corrective Action:

Valve repacked.
