Our September 22.1978 Part 21 Report indicated we would evaluate the cdequacy of the program, described as being undertaken on th2 H111 stone III unit undergoing a 300 start test at that time, after completion of the test.

Our Engineering and Quality Assurance Copartments have analyzed the injection pump failures. It is their conclusion that the failurcs were the result of oae or r. ore of the folloaing conditions.

1 Surface irregularities on barrel an#or plunger.

2 Incorrect material.

3 How punps (i.e.. no previous running).

After completion of the 300 start tests, all injection pumps were rccoved froin both P.111 stone III units. All pumps were inspected to newly estab-11shed criteria reflecting the above three areas of concern. All pumps were disassembled plungers and barrels were inspected and clearances recorded. Revised de-burring and stoning procedures were carried out on the helix on the plunger and the fuel inlet port on the barrel. In addition, one runn from each unit was rerun for 48 hours5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br /> and re-examined.

The results of the reruns were satisfactory.

Colt's conblusion is that all of the Hillstone III injection ptraps are now adequately run-in and are in good condition. No further action is contcaplated and both units are ready to go into service.
