Our SeptcMer 22,1970 Part 21 Report indicated we would evaluate the adcquccy of the program, described as being undcrtcken on the l'illstone III unit undergoing a 300 start test at that time, after ccmpletion of

Our Engincoring and Quality A:suran;;c Departments have analyzed the injcction pump failures. It is their conclusion that the failures were the result of one or rare of the folluning turiditiviih.

1 Surface irregularitics on barrel and/or plunger.

2 Incorrect raterial.

3 New pu:1ps (i.e. , no previous runntrig).

After completion of the 300 start tests, all injection pumps were removed from both Millstone III units. All pum,*s were inspected to neuly estab-lithad criterid reflecting the atiove three areas of concern. All pumps were discssembled. plungeis and barrels were inspected and clearances recorded. Revised de-burring and stoning procedures were carried out on the helix on the plunger and the fuel inlet port on the barrel. In addition, one puns from each unit was rerun for 48 hours5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br /> and re-exactned.

The results of the reruns were satisfactory.

' Colt's conclusion is that all of the N111 stone III injection pumps are now adequately run-in and are in good condition. Ho further action is contemplated and both units are ready to go into scryice.

Our September 22, 1978 report indicated the two Duquesne Light Co.,

Beaver Valley units in storage at Beloit would be dispositioned relative to the reported potential defect prior to shipment.

All injection pumps on the two Beaver Valley II units have been removed from the engines, inspected to the newly established criteria delineated in the referenced Supplemental Report #1, and reinstalled on the engines.

All of the Beaver Valley injection pumps are now adequately run-in and in good condition. No further action is contemplated and both units are ready to go into service.
