Anchorage

1. Is the anchorage configuration verification required (i.e., is the item one YZ NEI of the 50% of SWEL items requiring such verification)?

P222 is cantileveredoff E201. P221 anchorageis verified by rule-of-box by verifying the anchorage of E201. E201 anchorageis verified using the SEWS sheet for that piece of equipment.

2. Is the anchorage free of bent, broken, missing or loose hardware? YE NEIl UIZJ N/AEl

(Note: This question only applies ifthe item is one of the 50% for which an anchorage configuration verification is required.)

P222 is cantileveredand adequatelysecured off E-201. Anchorage for P222 is verified using the SEWS package for E-201. It is noted in the SEWS for E-201 that foundation drawing C-61 shows 6 Y2" J-bolts, length = 18". Only 3, Y2" bolts are requiredand were verified in the field.

Three Y2" bolts were verified by walkdown engineers.

X203 is not directly labeled in the field. Label request was enteredin eSOMS to have the item label created.

A contamination suit booty was found lying in the skid. This is noted as a housekeeping,non-seismic item. It was reportedto Operationsfor suggested removal.

The turbine and the pump are on different skids. However the concrete pedestalsare quite rigidand are tied together. Therefore there is no potentialfor differential movement between pump and driver.

This was verified by the SEWs package.
