- 1. Does anchorage of equipment in the area appear to be free of potentially YZ NE] UE-N/ARl adverse seismic conditions (if visible without necessarily opening cabinets)?
A step plate on the east side of the diesel generator has two fasteners in place and two missing. The connection is still sufficient even with the fasteners missing, as the plate support is welded to the floor; therefore, it is not considered a seismic hazard. CR-PNP-2012-04588 was written and WR 287230 initiated to replace the fasteners. See attached photo.
A missing fastener was found on the cover of C-227. This is not considered a seismic hazard; however, CR-PNP-2012-04589 was written and WR 287230 initiated to replace the fastener. See attached photo.
The steel curb between the DG A and B rooms has four empty bolts holes between the curb and the wall. It is judged that no bolts were ever in place in these locations. In addition, the sealant between the curb and the wall is in good condition. Therefore, it is not considered a seismic hazard.

Several Wej-lt anchors on the north wall do not have full thread engagement. While most have at least 75% engagement, the anchor on the support for 38-HO-107B has two threads disengaged, approx. 50%
engagement. Appendix C of Specification CI 10-ER-Q-E3 states that due to problems with some Wej-lt anchor installations, these anchors were tested and replaced as necessary, and those found acceptable were left in place. Due to the fact that these anchors have previously been evaluated, and that nut engagement does not control the design strength of a Wej-lt anchor bolt, this is not considered a seismic hazard. There is oil accumulating under the lube oil circulation pump at the southwest comer of the diesel generator. A small amount of the oil has accumulated on the floor as well. This is not considered a seismic interaction hazard; however, it was communicated to the system engineer/area owner. This is a previously identified condition with CR-PNP-2011-04187 and WO 00290562 initiated to address the concern.

There is a Y2"-diameter bolt missing on a Unistrut support that is behind, but not clamped to a conduit running from flooding alarm 30-LS-9029.
The span between the adjacent supports in good condition is approximately 9 feet, which is acceptable per Specification C-90-ER-Q-E5. Therefore, this is judged not to be an adverse seismic condition.

significant degraded conditions?
Surface corrosion and some scaling were found on service water inlet valve 29-HO-3837. There is no apparent material loss, so the valve is judged to be acceptable. See photo "PNPS 10-4-12 TI 040" in Share Point.
The base plate of a support for the RBCCW Suction Header piping is slightly lifted on one comer, and some of the grout is chipped off EC-24965 has been issued and will resolve the concern. See photo "PNPS 10-4-12 TI 041" in Share Point.
The grout pad under the pipe support for the P-202D non-safety related drain line is missing some material around its outer perimeter. As the support is beyond the safety class boundary at valve 30-HO-51, and the anchor bolts and majority of the grout are in place and appear sound, this is not considered a seismic hazard. CR-PNP-2012-04832 was written and WR 288870 was initiated to address the concern. See attached photo.

Tools were found in an open section of tube steel from pipe support H-30-1-1224. This is a general plant housekeeping concern, not a seismic interaction hazard, and was reported to Operations for suggested removal. See CR-PNP-2012-04411.

A chip and a crack were found on the corner of the concrete wall south of the TBCCW heat exchanger. In addition, a crack approx. 1/8" wide was found in the grout pad for a removable steel wall assembly, also south of the TBCCW heat exchanger. The cracks are judged to be superficial and have no impact on the load bearing function of both the concrete wall and the grout pad. CR-PNP-2012-04404 was written to initiate WR 286053 for repairs. See attached photos.
Minor degradation of the coating on RBCCW 16"-HE-30 suction header was observed in the area of the north end of the B-Aux Bay. There is some bubbling of coating and evidence of minor corrosion beginning.
Deficiency Request # 9537 was initiated to track/resolve this issue. See photo "PNPS 10-4-12 T1 043" in Share Point.

Non-safety related, HPCI gland seal condenser drain piping is piped through permanent scaffold rack 10-24 in the HPCI quad. Since the piping is not safety related and is well supported, this is not a seismic concern. CR-PNP-2012-04438 was written to address this item.
In addition to the stored scaffold, there are also three ladders in the upright position being stored in the area. If the ladders could be laid on their side, it would be less of potential hazard.
There are also multiple barrels of scaffold clips next to the scaffold cart which have lids lying loosely on one of the barrels. While not a seismic hazard, properly placing the lids on the containers would be good housekeeping practice. It was reported to Operations for suggested action.

Two separate temperature sensors, one in the HPCI room and one along the wall next to the stairs coming out of the HPCI quad, are hanging freely from the casing. The sensor numbers are: 24R-TS/D-43-201A, 24R-TS/D-44-201B (Both part of box J 2160) and 24R-TS/D-47-204C, 24R-TS/D-48-204D (from box J 2161). This issue was previously identified and Work Request 260250-01 was written to address it.

Two of the components we inspected are missing tags, pump P203 and turbine X203. In addition, we noted a broken tag near the SE comer of the room, label CK-2301-??? "Torus Suction Line Check Valve". A request was made in eSOM's for new tags.
Temperature switch 23-TS-2300-13 is in contact with the insulation jacket of piping above the switch.
Based on review of Pilgrim Q-List, ENN-MS-S-009-PNP, and HPCI Design Basis Document, SDBD-23, this component does not perform a safety function. The switch only performs an alarm function and does not actively control any equipment or process. The temperature element sits in a well in the lube oil piping and therefore does not constitute part of the lube oil pressure boundary. The pipe in contact with the switch is well supported such that relative movement between the pipe and temperature switch during a seismic event is expected to be small. There is enough flexibility in the switch assembly and pipe insulation to accommodate the movement without adverse effect. It is therefore concluded that this does not represent an adverse seismic interaction.

A missing anchor bolt was found on the base plate of PS-3829A, as well as a missing equipment tag. This missing anchor bolt is demonstrated to be acceptable in the Screening Evaluation Work Sheet (SEWS) for PS-3829A, dated 05/06/1998, under USI A-46. See attached photo and LB Evaluation 001. The equipment tag was reported to Operations, and an ESOMS label request was initiated. See CR-PNP-2012-04411.
- 2. Does anchorage of equipment in the area appear to be free of significant degraded conditions?
An apparently abandoned, vertically-running conduit was found directly west of P208E, and was not labeled as abandoned. The conduit has a broken/corroded mounting clamp below the grating, as well as ground wire extending from the top of the conduit. The conduit is still securely fastened to the wall with other mounting clamps that are in good condition, so it does not present an adverse seismic interaction concern. CR-PNP-2012-04377 was written to address the issue. See attached photo.

There are several two-bolt base plates with two additional empty holes near the top of the plate. Upon further inspection, it can be seen that the plate is welded to a steel support at the top, and so it is not likely that two bolts are missing. Therefore, this is not considered an adverse seismic condition. See photo "PNPS 10-9-12 TI 023" on Share Point.
There is a base plate at approx. EL 15' on the east wall supporting the stairs that has a bolt hole with no anchor, and the anchor on the opposite side is skewed. It can be seen that ear plates are welded to the top of the main base plate in order to avoid bolting to the masonry block-out, where the original bolt holes were located. The two bolts in the ear plates are in good condition; therefore, this is not considered an adverse seismic condition. See photos "PNPS 10-9-12 TI 024" and "PNPS 10-9-12 T1 025" on Share Point.
There is a base plate at approx. EL 5' on the east wall supporting the stairs that has a nut in place with no anchor. Drawings A-35 and C-356 SH 1 show that anchorage for stair 5 was typically designed to consist of two Y4" diameter expansion anchors. In the field, multiple variations of this anchorage can be found. In general, the as-built anchorage is judged to be acceptable, with welds to embedded plates noted in particular to be more robust than the original design. Also, this particular clip angle has an additional anchor in place when compared to the drawings, and so is not considered a seismic hazard.

There are two base plates at approx. EL 5' and EL 15' on the east wall supporting the stairs that have one bolt hole with no anchor. Upon further inspection, it can be seen that the side of the plate without a bolt is welded to an embedded plate on the concrete. Therefore, this is not considered an adverse seismic condition. See photo "PNPS 10-9-12 TI 032" on Share Point.
The base plate for pipe support H 10-1-46SS along a 20" RHR pump line near MO-1001-43C is on the east wall at approx. EL -3'. It consists of a base plate with two bolts in place and two empty bolt holes, plus an additional plate welded to the original base plate with two bolts. This is shown to be an intentional design per drawing H10-1-46SS, Rev. El. Therefore, the empty bolt holes are not considered an adverse seismic condition. A scaffolding pole was found in open tube steel on a pipe support approx. 8 feet west of valve 30-HO-88. This is not considered a potentially adverse seismic interaction; however, Operations was notified for suggested removal of the item, and CR-PNP-2012-04472 was written. There is a long, tight ( < 1/16" wide) crack in the concrete on the east wall running approximately from EL 3' to EL -3'. The crack was discussed with the structural monitoring program engineer and is judged to be insignificant and will not impact the structural integrity of the concrete wall; therefore, this is not a potentially adverse seismic condition. See photo "PNPS 10-9-12 TI 033" in Share Point.
Two conduits, A3068 and A3069, passing through the south wail have degraded concrete at the penetration, approx. Y" in depth. This is determined to be surface degradation only, as no rebar is exposed and the wall appears to be structurally sound over the remaining surface.
Additionally, the degradation does not affect the clearance between the concrete and the conduit, and does not create a pathway through secondary containment. This is judged not to be an adverse seismic condition; however, CR-PNP-2012-04466 was written and WO 00329254 was initiated to repair the degradation. See attached photo.

Safety related piping from MO-1301-61 is supported by unistrut from a floor support. The support was noted to have missing grout beneath the base plate. Review of the support noted that it is designed for vertical forces only, and although grout is missing, the anchor capacity is not compromised. Therefore this is not a seismic concern, but as good practice, CR-PNP-2012-04622 was written to replace the grout to its intended original design.

Trash items were noted at the entrance to the room. This is a general plant housekeeping concern and not a seismic interaction hazard. It was reported to Operations for suggested removal.

Loose tools were noted to be next to standby liquid control test tank. This is a general plant housekeeping concern and not a seismic interaction hazard. It was reported to Operations for suggested removal.

The base plates on the west HCU upper gallery support steel frame have several anchor bolts without full thread engagement, but none less than 75% engagement. As the bolts are Wej-lt expansion anchors, the strength of which is not governed by full thread engagement of the nut, the state of these anchors is judged sufficient to develop the full design strength of the anchor. Therefore, it is not considered an adverse seismic condition.
Two LLRT Type B racks were found to have missing bolts. Penetration 52 is missing one of four bolts, and penetration 5 1B is missing two of four bolts. These racks were determined to be non-safety related, and have sufficient anchorage to support the low mass of the piping. Each rack has at least one bolt in both the top and bottom base plates.
Therefore, this is not considered an adverse seismic condition. See photo "PNPS 10-11-12 TI 048" on Share Point.

Two light fixtures above the B-18 enclosure are hung by chains that are draped over and offset by a cable tray. The lighting fixture is approx. 30 pounds, which results in a 15-pound load per chain. The worst case angle of the chain is approx. 45 degrees, and would result in a potential horizontal load of approx. 10 pounds on the tray at each point of contact. This load is judged to be insignificant and will not impact the load carrying capacity of the cable tray. Therefore, it is not considered to be an adverse seismic condition. Radiation blankets shielding the SDIV are hung on open S-hooks.
However, vertical acceleration will be low and there will be no net uplift due to the high mass of the blankets. Horizontal movement will also be minimal due to damping and long pendulum behavior. Therefore, this is not considered to be a potentially adverse seismic interaction. See photo "PNPS 10-11-12 TI 030" on Share Point.
Hose reel RB-07-23 is close enough to panel C-165 to potentially swing into the panel during a seismic event. However, it was discussed with Operations and they determined that there is no sensitive equipment inside the panel. Therefore, this is not a seismic interaction concern.

The light fixture above the south door of the B-18 enclosure is hung by an open S-hook. As the fixture could potentially fall or swing into equipment in the room during a seismic event, CR-PNP-2012-04513 was written and WR 286905 initiated to address the concern.

An un-tethered wrench and notebooks were found adjacent to HCU 06-27 at the south end of the rack. Other tools are securely hanging on a clip next to the loose wrench, but the wrench does not have a loop for the clip to attach to. This is determined to be deliberate storage of the wrench and notebooks, and is not considered a housekeeping concern.
There is also no sensitive equipment around the items were they to fall during a seismic event, so this is not a seismic interaction concern. See photo "PNPS 10-11-12 TI 011" on Share Point.
A box containing extra charts was found resting on temperature recorder 30R-TR-9013/9014. This is determined to be deliberate storage, and is of low enough mass to not be a seismic interaction concern. See photo "PNPS 10-11-12 TI 044" on Share Point.
A sign for a T-wrench and what looked to be a loose bar were found on the ground. Upon closer inspection, the bar is attached to the T-wrench, which is properly secured to the wall and is not a seismic concern. HCU 26-51 has one 5/8" expansion anchor bolt, different from all the other 1/2" Unistrut bolts. This is not considered to be an adverse seismic condition. See photo "PNPS 10-18-12 T1 026" on Share Point.
The base plates on the east HCU upper gallery support steel frame have several anchor bolts without full thread engagement, but none less than 75% engagement. As the bolts are Wej-lt expansion anchors, the strength of which is not governed by full thread engagement of the nut, the state of these anchors is judged sufficient to develop the full design strength of the anchor. The cable way box below panel B-16 9L has 2 missing fasteners and 14 in place. While this is not considered a seismic hazard, CR-PNP-2012-04684 was written and WR 287697 initiated to address the concern. See attached photo.

Loose lead bricks are stacked adjacent to LLRT Type B Penetration 201B and two electrically traced lines. The LLRT rack is non-safety related, and the piping lines were also determined to be non-safety related using drawings M239 SHI, M1004 SH 305-306, and H-49 051. In addition, the piping was determined to be Sch. 80 stainless steel, and so is judged to be sufficiently robust that the lead bricks do not present a seismic interaction hazard.
