Operations does not allow this panel to be opened while in service and it very rarely is taken out of service. Additionally, the arrangementof the panel is such that it would require extensive supportby Maintenance Dept to gain access to the cubicles and the lower internalspace where anchorageis located (dozens of threadedfasteners). This is further complicated by the fact that the entire panel is housed within a relatively tight fitting environmental enclosure that makes such access even more challenging. This panel is therefore considered to be not accessible for intemal inspectionper the methodology/criteriaof this inspectionprogram.

Instructions for Completing Checklist This checklist may be used to document the results of the Seismic Walkdown of an item of equipment on the SWEL The space below each of the following questions may be used to record the results of judgments and findings. Additional space is provided at the end of this checklist for documenting other comments.

Anchorage

1. Is the anchorage configuration verification required (i.e., is the item one Y[E] NZ of the 50% of SWEL items requiring such verification)?

Although the panel is anchored,the reardoor is inaccessible, therefore it was not chosen for the 50% verification.

2. Is the anchorage free of bent, broken, missing or loose hardware? Y[ NE]I UE N/AE]

The front of panels A5-2 and A5-9 are missing a single bolt in each. An eye hook on the back of panel A5-2 is not engaged and is hanging out of the panel. The back of panel A5-4 is missing an eyehook. CR-PNP-2012-4535 was written.

A small screw inside of the panel, connecting a bracket to a wire chase is missing. This is a redundantbracket and it is holding a small chase of wires, therefore, this does not pose any hazard.

3. Is the anchorage free of corrosion that is more than mild surface Y] NEI UE] N/AE-]

oxidation?

On anchoragethat is visible within the cabinet, it is free of corrosion.

4. Is the anchorage free of visible cracks in the concrete near the Y[ NE] UE] N/AE]

anchors?

Visible anchorageat the front of the panel is free of cracking in the concrete.

6. Based on the above anchorage evaluations, is the anchorage free of YZ NEI] U]

potentially adverse seismic conditions?

Each panel is bolted together as verified in the SEWS package, therefore not a seismic hazard.

Small tag stating "ManualClosed" is attached by two small screws.

One small screw is missing but the tag is still present. This is not a seismic concern and since the tag is still intact, adequate.

Interaction Effects

7. Are soft targets free from impact by nearby equipment or structures? YD- NO UD- N/AD-Inside panel, components are adequatelysecured. The outside of the cabinet is robust and free of interactionfrom other equipment or systems.

Operationlocker next to A5 is not adequatelysecured to the floor or wall as visually inspected from the outside and could fall into nearby panel during a seismic event. The locker is approximately4.5' away from A5. The locker is approximately 6' tall. CR-PNP-2012-04544 is written to address this seismic concern. A 1.17.1 proceduralevaluation was performed as action to remedy seismic concern.

8. Are overhead equipment, distribution systems, ceiling tiles and lighting, YO ND] UD- N/A[-D and masonry block walls not likely to collapse onto the equipment?

There are no block walls in the immediate area and cable trays are adequately filled.

9. Do attached lines have adequate flexibility to avoid damage? YZ NDI UD- N/AD-
10. Based on the above seismic interaction evaluations, is equipment free YE] NO ur-of potentially adverse seismic interaction effects?
