Detailed inspections were performed and photos taken of 2 of the 145 C V305-127 valves: HCU 26-39 (West) and on HCU 42-19 (East). The remainderof the CV305-127 valves were inspected for overall configuration compliance along with their HCU assembly as partof the area walk-by. Note that this included a detailed visual inspection of the anchoragesof all HCUs.

Anchorage

1. Is the anchorage configuration verification required (i.e., is the item one Y[-7 NZ of the 50% of SWEL items requiring such verification)?

This is an anchored component, but due to lead shielding, one of the anchorbolts cannot be visually inspected, therefore, this is not part of the 50% anchorage verification.

2. Is the anchorage free of bent, broken, missing or loose hardware? YE Nr7 UD7 N/A[l Three visible anchors are in good condition.
3. Is the anchorage free of corrosion that is more than mild surface YE NDl U[-] N/A-oxidation?

Three visible anchors are free of oxidation.

4. Is the anchorage free of visible cracks in the concrete near the YN ND UD- N/A[-D anchors?

A surface crack is visible near one anchor, but since all bolts are cast in place in the foundation, this is not a structuraldeficiency. Although one anchorbolt and nut is not visible, the concrete next to this and all other anchors is visible and free of cracking.

Area walk-by is AWC-04.

P205 is not directly labeled in the field. Label request was entered in eSOMS to have the item label created.

The turbine and the pump are on different skids. However the concrete pedestals are quite rigid and are tied together. Therefore there is no potentialfor differentialmovement between pump and driver.

This was confirmed by the SEWs package.

Anchorage

1. Is the anchorage configuration verification required (i.e., is the item one YI- NO of the 50% of SWEL items requiring such verification)?

Due to lack of visual verification of rearcabinet anchors, this was not chosen for the 50% verification.

2. Is the anchorage free of bent, broken, missing or loose hardware? YO NE!I UiE N/All Anchorage at the back of the panel is not visible without breaking the plane of the cabinet, but anchorsat the front of the cabinet are in good condition.
3. Is the anchorage free of corrosion that is more than mild surface YE NE! UL- N/AZ!

oxidation?

Anchor bolt on outside of cabinet securing a clip is corrodedand the threads of the anchor are no longervisible. CR-PNP-2012-04509 was written to correct the issue. The corrosion is only on the surface with no apparentloss of metal thickness. Therefore there is no loss of strength and the hold down plates will perform their design function. The one anchorbolt is fused to the nut by general but localized corrosion.

Although they are fused there is no flaking of metal or any apparent reduction in bolt or nut diameter. Therefore there is no loss of strength and the anchorwill perform its design function.

(Note: This question only applies ifthe item is one of the 50% for which an anchorage configuration verification is required.)

Refer to question #1.

6. Based on the above anchorage evaluations, is the anchorage free of Y[ NEI UEr potentially adverse seismic conditions?

This component was a previous outlier for IPEEE. The addition of clips, verified in the field, was completed. Refer to question #3 above for explanation of bolt corrosion at clip.

Interaction Effects

7. Are soft targets free from impact by nearby equipment or structures? YZ NEI Ur- N/A[:]

Maintenance tools (flashlight, screwdriver, etc.) are located next to the panels, but per procedure 1.17.1, this is acceptableplacement and does not pose a seismic hazard.

8. Are overhead equipment, distribution systems, ceiling tiles and lighting, YN NEI Un N/AD and masonry block walls not likely to collapse onto the equipment?

This component is locatedin a small enclosure specifically designed for this equipment. It is in good condition. Light bulbs within the cabinets are plastic coated and do not pose a seismic hazard.

9. Do attached lines have adequate flexibility to avoid damage? Y[ NEJ U-- N/AD--
10. Based on the above seismic interaction evaluations, is equipment free Y[ NE-I U -

A small plastic hose next to the batteries inside of the panel was noted.

Due to restaintsof not breaking the plane of the cabinet while it is open, it could not be determinedif this is partof the battery assembly. It does not pose a seismic risk.

The paint is stripped aroundthe welds between the valve body and the piping. There is mild surface corrosionin the area, but this is not consideredan adverse seismic condition.

Under the valve, two pins were found duct taped to a stanchion above a spring can. This was discussed with operationsand engineeringmanagement, and was determined to be an acceptable practice for holding the pins in place; therefore, it is not a seismic interaction or a housekeeping concern.
