Therewerenooperational orsafetyconsequences orimplications attributed toR-32beinginoperable forgreaterthansevendays,becauseR-32wasstillfunctioning andcapableofresponding toradiation levelsat104oftheTSlimits,insteadofthedesired14ofTSlimits.Becausethesetpointisveryconservative tobeginwith,theonedecadeerrorinthecalibration constantstillallowedthesystemtorespondwellwithintheTSlimits.ThedesignforR-32isthatwhentheoutputreaches0.1mr/hr,recorderRK-47Aactivates toprovideindication andrecording ofR-32intheControlRoom.R-32was.functioning correctly, butduetothistransposition error,theoutputwasinerrorlowbyonedecade.(ThedetectorforR-32continued tofunctionasdesigned, andcontinued todetectandmonitorradioactivity.
Thedetectoroutput,asmeasuredinCPMwasstill,correct.However,the.outputreading,"
as'expressed inmr/hr,wa'sone'ecade toolow.)Therefore, theRK-47ArecorderintheControlRoomwouldnothaveactivated untiltheactualradiation reached1mr/hr(onedecadehigherthandesired).
Thiscorresponds toareleaseconcentration ofapproximately 1E-02microcuries percubiccentimeter (uCi/cc)noblegasesorareleaserateofapproximately 1.33E-02curiesper'econd(Ci/sec).

Whenthecalibration constantwascalculated (onOctober7,1993),theI&Ctechnician misplaced thedecimalpointwhiletransposing thecalculated valuefromdecimalnotationtoscien-tificnotation.,

Consequently, anincorrect poweroften(onedecadetoolow)wasrecordedontheprocedure datasheetandeditedintotheR-32controlterminal.

"HighRangeEffluentMonitorsR-12A,R-14A,R-15A,R-31,R-32",wasthenchangedbytheI&Ctechnician, onOctober7,1993,toreflectthetol'erance bandofoutputreadingsforR-32,basedonthenewvalueofthecalibration constant.

PT-17.5wasperformed, andthePlantProcessComputerSystem(PPCS)wascheckedtoconfirmthattheou'tputofR-32waswithinthePPCS"qualitycode"and"currentvalue"tolerances.

Furtherinvestigation revealedthatthisinconsistent valuewascausedby.thetransposition errordiscussed above.?TheengineernotifiedtheI&Cgroupofhisdiscovery.
