By this letter, AAF International
("AAF") provides notice to the U.S. Nuclear Regulatory Commission
("NRC") of a possible deviation or defect in nuclear qualified High Efficiency Particulate Air ("HEPA") filters manufactured by AAF. Although late, this interim notification is submitted pursuant to the requirements of 10 CFR Part 21.On December 5, 2012, AAF received a report from an end user of its HEPA filters, which indicated that two (2) nuclear qualified HEPA filters failed a Resistance to Pressure test conducted by or on behalf of the end user. The cause of the test failure is not known as of the date of this interim report, but AAF is in the process of investigating the matter and performing a root cause analysis.
In order to complete the root cause analysis, it is necessary for AAF to build several filters for testing. The process of building new filters and testing will likely require approximately thirty (30) days due to the time necessary to procure raw materials from AAF's suppliers.
However, AAF anticipates that its investigation will be completed no later than April 15, 2013.Attached hereto, AAF provides a list of customers who may be impacted by the potential defect in these nuclear qualified HEPA filters. AAF will update this list upon completion of its investigation into this matter. AAF will also follow up with a full report to the NRC upon the completion of its investigation.
