Interim Report of a Deviation or Failure to Comply Associated with a Limitorque Actuator

In accordance with the requirements of 10 CFR 21.21(a)(2), the Tennessee Valley Authority is submitting this interim report for Sequoyah Nuclear Plant (SQN), Unit 1.

On October 19, 2022, during as-left testing of the Unit 1, turbine-driven auxiliary feedwater pump, trip and throttle valve (actuator, QA 1, motor operated valve, Limitorque model SMB-000) a motor (the motor is a sub-component of the valve actuator) anomaly was identified. The motor test revealed higher than expected current and the motor was running at higher-than-expected RPMs. The increased current appeared to be associated with the motor shunt winding. On December 1, 2022, SQN completed its 10 CFR Part 21 discovery process and determined the need to perform a 10 CFR Part 21 Evaluation.

The motor was returned to Flowserve (at Framatomes request) for evaluation. The vendor evaluation is ongoing and will not be complete in time to support the requirement to complete SQNs Evaluation within 60 days. Because the 10 CFR Part 21 Evaluation cannot be completed within 60 days of discovery, SQN is submitting this interim report.

SQN intends to complete the 10 CFR Part 21 Evaluation no later than March 1, 2023.
