Interim Report of a Deviation or Failure to Comply Associated with Check Valve 0-CKV-023-0565 for D1 Residual Heat Removal Service Water Pump In accordance with the requirements of Title 10 of the Code of Federal Regulations (10 CFR) 21.21(a)(2), the Tennessee Valley Authority is submitting this interim report for Browns Ferry Nuclear Plant (BFN).
On August 10, 2023, following completion of maintenance on the D1 Residual Heat Removal Service Water (RHRSW) Pump and its check valve 0-CKV-023-0565, the clearance was removed. During the removal, header pressure dropped to 20 psig bringing in the "RHRSW Header Pressure Low" alarm and Unit 1 operators started the D2 RHRSW Pump to restore pressure. Initial thought was an air pocket caused the pressure drop.
Following completion of the tagout, the D2 RHRSW Pump was secured and pressure dropped to 20 psig again. Unit 1 operations personnel restarted the D2 RHRSW Pump again to restore header pressure. An Assistant Unit Operator (AUO) was dispatched to walkdown the D1 RHRSW Pump alignment and found the pump rotating backwards. The AUO closed the D1 RHRSW Pump discharge valve as directed by the Senior Reactor Operator (SRO) and rotation stopped. Then the AUO slowly re-opened the discharge valve of the D1 RHRSW Pump and the backwards rotation began immediately. BFN completed its 10 CFR Part 21 discovery process and determined the need to perform a 10 CFR Part 21 Evaluation. The vendor for testing (Watts Water) was contacted and will assume responsibility for performing the Part 21 Evaluation for the check valve. This Part 21 Evaluation is being tracked by TVA under Condition Report 1873788.
The check valves were returned to Watts Water for evaluation. The scheduled work to replace the remaining check valves was postponed until the evaluation can be completed.
The vendor evaluation is ongoing and will not be completed in time to support the requirement to complete the evaluation within 60 days. Because the 10 CFR Part 21 Evaluation cannot be completed within 60 days of discovery, BFN is submitting this interim report. The vendor has expressed that they are tracking the Part 21 evaluation under
U.S. Nuclear Regulatory Commission Page 2 October 6, 2023 tracking number 452830 and the due date for the evaluation to be completed is November 1, 2023.
