LER SUPPLEMENTAL INFORMATION BQRO-50-327/8002 Technical Specification Involved 3.3.3.9 Reported Under Technical Specification 6.9.1.12.b Date of Occurrence: 3/6/80 Time of occurrence: 1000 CST Unit 1 Identification and Description of Occurrence:

Engineering personnel in reviewing surveillance instructions noted that the contents of the task decontamination collector tank was discharged via the liquid radwaste effluent line to the cooling tower blowdown with the flow elements in both lines inoperable without flow estimates being performed. This is in violation of Action 30 of Table 3.3-12 of LCO 3.3.3.9.

Conditions Prior to Occurrence:

Unit 1 in mode 6 loading fuel for first core load.

Action specified in the Technical Specification Surveillance Requirements met due to inoperable equipment. Describe.

N/A Apparent Cause of Occurrence:

Misunderstanding of Technical Specification requirements.

Analysis of occurrence:

There was no hazard to the public health or safety. SI-400, Release Permit, was performed and verified no activity in the tank.

Corrective Action:

Personnel instructed to observe requirements for inoperable monitor.
