The panel covering removable opening in masonry block wall 64.4 has a nut that is loose! not fully engaged. There's also a lose anchorbolt lying in the channel that mounts the braces to the floor. This is not a seismic concern as it's a robust design with redundantbolting.

significant degraded conditions?
Mild surface oxidation noted on pump C-1 19 frame.

A sprinklerhead is within two inches of the bottom of cable tray, but is adequately covered with a guard, therefore acceptable.

Scaffolding is currently being erected below the valves MO 1400-24B and MO1400-25B to allow access to them. This is a work in progress and is tagged appropriately. All extra equipment in the area is properly secured and cart wheels are choked.

A fasteneris missing on the gaitronicscover next to instrument racks C2205A and C2205B. This is not a seismic hazard as the remaining three fasteners adequately secure the cover.
CR-PNP-2012-04657was written to address the missing fastener.
The cage! fence around the sample sink is missing a single set screw and what appears to be a mounting anchoron one of the posts. The mounting anchoris located in the walking path of the dooropening and therefore is a trip hazard, and so its exclusion was likely deliberate.
There are redundantfasteners the length of the fence and this is not a seismic concern and therefore is acceptable.
CR-PNP-2012-04659 was written to address the missing set screw.

Dooron 52-303 is slightly open and two securing nuts appearloose.

PrintersLPO1, LP02, and LP03 are located on two stands bolted together. The aspect ratio of the entire assembly is less than 2:1 and it is located 66" from the nearestpanels. A computer cabinet adjacent to these printershas an aspect ratio of approx. 2:1, and is 118" from the nearestpanels. These do not present a seismic interactionconcern per Procedure 1.17.1, Rev. 12. A bookshelf is located between CP600 and a window frame, in contact with the panel and approx. 1" from the window frame. The bookshelf cannot fall forward due to the desk directly in front of the shelf; however, it could rattle into CP600. Therefore, the ladders could not be verified to be secured. However, based on the camera inspection, these ladders are not considered an adverse seismic interactionbecause the area is relatively free of equipment and the manner in which they are positioned will likely not lead to tipping into equipment.
The general housekeeping in the RWCU HX room on ReactorBuilding 51' is below standard. There are hoses, cords, tools, rope, and misc debris on the floor. None of the items were observed to present a seismic interactionhazard or have an immediate adverse impact on the equipment in the room. CR-PNP-2012-04739was written to address this item.

A base plate supporting drainpiping was noted to be missing anchor nuts and to be in poor condition. The support appears to have once supporteddrain line piping, but now only sits next to the piping. CR-PNP-04469 was written to address the item.
Evaluationby the Seismic Walkdown Engineers in accordance with EN-DC-168 concluded that this condition does not introduce an adverse seismic interaction. The very compact support assembly will remain in place during a seismic event due to the anchorstuds and two existing anchornuts. The drain piping is approximately 6 inches off the floor near the west wall, not in close proximity to any vulnerable components. Itis socket welded construction and otherwise well supportedsuch that displacements in a seismic event are expected to be less than 2 inches.

The set screw is stripped for TSD-45 & TSD-46. With strippedor missing set screws, the temperature switches are not restrainedvertically by the dogbone mounting sleeve. - 7. FSAR Appendix C.2, Rev.28 Licensing Basis As defined in FSAR 12.2.1.1, these shielding blocks are considered to be Class II SSCs. The FSAR specifically states "Class II designated structures and/or equipment shall not degrade the integrity of any structures and/or equipment designated Class I". This is what is typically referred to as a potential seismic Il/I interaction. The blocks must be prevented from becoming missiles during a seismic event and adversely impacting safety-related (Class I) equipment in the area.
In addition to seismic loading, the shielding blocks must also be restrained when subjected to loads/pressures associated with postulated pipe breaks. FSAR Appendix C.2 lists the various breaks that have the potential to affect the bioshield wall blockouts, including the NI B nozzle location. The penetration blockout restraint steel was design for the maximum pressures resulting from a credible pipe break such that the shielding blocks will not become missiles during a pipe break transient and adversely impact safety-related equipment in the area.
Evaluation During the walkby it was observed that there were welded steel hooks to accommodate removable restraint steel for this blockout but the restraint steel was not in place. Review of plant documentation revealed that these blocks should have been removed during RFO15 (2005) when the restraint steel and all of the blocks in the upper portion of this penetration were removed per Plant Design Change ERCN 04114532 and Work Order WO51527937-01. Extensive searching of plant records found no documentation that authorized or justified leaving the unrestrained shielding blocks in the penetration opening.
