Thereactorcoolantsystem(RCS)wasbeingmaintained atatemperature ofapproximately 547degreesFandaPRZRpressureofapproximately 2235PSIGinpreparation foramodechangetoMode2.The"A"motor-driven auxiliary feedwater (AFW)pumpwasoperating tomaintainwaterinventory inbothsteamgenerators (S/G)usingtheflowpaththroughcross-connect valve4000A,andthe"B"motor-driven AFWpumpwasnotoperating.
Thecircuitbreakerforthe"A"mainfeedwater (MFW)pumphadjumpersinstalled andwasclosedinthe"test"position, tocomplywithtemporary administrative requirements.
TheControlRoomoperators wereintheprocessofrealigning thecondensate andMFWsystemsfromthelineupusedfor"CleanupRecirculation" ofthesesystemstothenormalat-powerlineup.Whenin"CleanupRecirculation" thereiscondensate flowtothehotwellwhichprovidesadifferential pressure(D/P)betweenthecondensate pumpdischarge andtheMFWpumpsuction.Whenrealigning tothenormalat-powerlineupwithoutMFWflow,theD/Pmaydecreasetozeroduringtherealignment.
Atapproximately 1929EDSTonJuly7,1996,MFWpumpsealwaterD/Pdecreased belowthealarmsetpointof15PSID.ThisresultedinanalarmfromMainControlBoard(MCB)Annunciator H-11"FeedPumpSealWaterLoDiffPress15PSI".TheControlRoomoperators referredtoAlarmResponseProcedure AR-H-11,but,sincenoMFWpumpswereoperating, noactionswererequired.

Atapproximately 1953EDST,the&#xb9;1Asealwaterboosterpumpwasmanuallystarted,whichincreased thesealwaterD/Pabove15PSID.Thisclearedthealarmcondition forAnnunciator H-11,whichallowedtheMFWpumpbreakertobeclosedinthetestposition, aspertemporary administrative requirements.
Whenthebreakerwasclosed(intest),theautostart signalfortheAFWpumpswasremoved.Subsequently, thesecondrunningAFWpumpwassecuredatapproximately 2003EDST.C.INOPERABLE STRUCTURES, COMPONENTS, ORSYSTEMSTHATCONTRIBUTED TOTHEEVENT:NoneD.OTHERSYSTEMSORSECONDARY FUNCTIONS AFFECTED:

Theunderlying causeoftheautostart wasthatthelogicforautostart wascreatedbytheopeningofthe"A"MFWpumpbreaker.ThiseventisNUREG-1022 CauseCode(E).Corrective actiontopreventrecurrence isoutlinedinSectionV.B.NACFOAM366(495)

Thereactorcoolantsystem(RCS)wasbeingmaintained atatemperature ofapproximately 547degreesFandaPRZRpressureofapproximately 2235PSIGinpreparation foramodechangetoMode2.The"A"motor-driven auxiliary feedwater (AFW)pumpwasoperating tomaintainwaterinventory inbothsteamgenerators (S/G)usingtheflowpaththroughcross-connect valve4000A,andthe"B"motor-driven AFWpumpwasnotoperating.

Thecircuitbreakerforthe"A"mainfeedwater (MFW)pumphadjumpersinstalled andwasclosedinthe"test"position, tocomplywithtemporary administrative requirements.

TheControlRoomoperators wereintheprocessofrealigning thecondensate andMFWsystemsfromthelineupusedfor"CleanupRecirculation" ofthesesystemstothenormalat-powerlineup.Whenin"CleanupRecirculation" thereiscondensate flowtothehotwellwhichprovidesadifferential pressure(D/P)betweenthecondensate pumpdischarge andtheMFWpumpsuction.Whenrealigning tothenormalat-powerlineupwithoutMFWflow,theD/Pmaydecreasetozeroduringtherealignment.

Atapproximately 1929EDSTonJuly7,1996,MFWpumpsealwaterD/Pdecreased belowthealarmsetpointof15PSID.ThisresultedinanalarmfromMainControlBoard(MCB)Annunciator H-11"FeedPumpSealWaterLoDiffPress15PSI".TheControlRoomoperators referredtoAlarmResponseProcedure AR-H-11,but,sincenoMFWpumpswereoperating, noactionswererequired.

TheControlRoomoperators observedtheautostart ofthe"B"AFWpump,andcontrolled AFWflowtothedesiredflowrateforMode3conditions.

Atapproximately 1953EDST,the¹1Asealwaterboosterpumpwasmanuallystarted,whichincreased thesealwaterD/Pabove15PSID.Thisclearedthealarmcondition forAnnunciator H-11,whichallowedtheMFWpumpbreakertobeclosedinthetestposition, aspertemporary administrative requirements.

Whenthebreakerwasclosed(intest),theautostart signalfortheAFWpumpswasremoved.Subsequently, thesecondrunningAFWpumpwassecuredatapproximately 2003EDST.C.INOPERABLE STRUCTURES, COMPONENTS, ORSYSTEMSTHATCONTRIBUTED TOTHEEVENT:NoneD.OTHERSYSTEMSORSECONDARY FUNCTIONS AFFECTED:

C.ROOTCAUSE:Theunderlying causeoftheMFWpumpbreakertrippingopenwasachangeinsealwaterdifferential pressurethatoccurredduringsystemrealignment.
