Thissituation wascreatedtobeincompliance withtheGinnaStationImprovedTechnical Specifications (ITS)Table3.3.2-1Function6.f.Topreventanunnecessary actuation ofbothAFWpumpsduringMode2conditions, aMFWpumpbreakermaybeadministratively closedinthetestposition, provideditiscapableofbeingtrippedonundervoltage andovercurrent conditions.
Theadministrative requirements wereestablished tocomplywithITSTable3.3.2-1,whichrequirestheautostart functionofthemotor-driven AFWpumpuponopeningofbothMFWpumpbreakersduringModes1and2.PriortoAmendment No.61totheITS,thisfunctionwasonlyrequiredinMode1whentheMFWpumpswereactuallyinservice.TheadditionofMode2(whentheMFWpumpsmaynotbeinservice)wasinadvertently addedtotheITSbyAmendment No.61.ThiseventisNUREG-1022 CauseCode(A),"Personnel Error".Thiserrorwasacognitive erroronthepartofaplantelectrician, whodidnotdetecttheinterference betweentherackingarmandtheactuating rod.Thiserrorwasassociated withactivities thatwerecoveredbyanapprovedprocedure, andwasnotcontrarytothatprocedure.
Theprocedural guidancewascorrect,butenhancements totheprocedure havebeenidentified.
Therewerenounusualcharacteristics ofthelocationofthisbreakerthatcontributed tothisevent.Notethatthisinterference canonlybecreatedwhenrackingthebreakerouttothetestposition.
Theautostart oftheAFWpumpdoesnotmeettheNUMARC93-01,"Industry Guideline forMonitoring theEffectiveness ofMaintenance atNuclearPowerPlants",definition ofa"Maintenance Preventable Functional Failure".

Thissituation wascreatedtobeincompliance withtheGinnaStationImprovedTechnical Specifications (ITS)Table3.3.2-1Function6.f.Topreventanunnecessary actuation ofbothAFWpumpsduringMode2conditions, aMFWpumpbreakermaybeadministratively closedinthetestposition, provideditiscapableofbeingtrippedonundervoltage andovercurrent conditions.

Theadministrative requirements wereestablished tocomplywithITSTable3.3.2-1,whichrequirestheautostart functionofthemotor-driven AFWpumpuponopeningofbothMFWpumpbreakersduringModes1and2.PriortoAmendment No.61totheITS,thisfunctionwasonlyrequiredinMode1whentheMFWpumpswereactuallyinservice.TheadditionofMode2(whentheMFWpumpsmaynotbeinservice)wasinadvertently addedtotheITSbyAmendment No.61.ThiseventisNUREG-1022 CauseCode(A),"Personnel Error".Thiserrorwasacognitive erroronthepartofaplantelectrician, whodidnotdetecttheinterference betweentherackingarmandtheactuating rod.Thiserrorwasassociated withactivities thatwerecoveredbyanapprovedprocedure, andwasnotcontrarytothatprocedure.

Theprocedural guidancewascorrect,butenhancements totheprocedure havebeenidentified.

Therewerenounusualcharacteristics ofthelocationofthisbreakerthatcontributed tothisevent.Notethatthisinterference canonlybecreatedwhenrackingthebreakerouttothetestposition.

Theautostart oftheAFWpumpdoesnotmeettheNUMARC93-01,"Industry Guideline forMonitoring theEffectiveness ofMaintenance atNuclearPowerPlants",definition ofa"Maintenance Preventable Functional Failure".

IV.ANALYSISOFEVENT:Thiseventisreportable inaccordance with10CFR50.73,LicenseeEventReportSystem,item(a)(2)(iv),whichrequiresareportof,"Anyeventorcondition thatresultedinamanualorautomatic actuation ofanyengineered safetyfeature(ESF),including thereactorprotection system(RPS)".ThestartofanAFWpumpisanactuation nfanESF.Anassessment wasperformed considering boththesafetyconsequences andimplications ofthiseventwiththefollowing resultsandconclusions:
