While in manual, the automatic flow demand signal decayed to near zero. When the error was discovered, the operator then failed to stop and notify supervision. Instead, he placed the 'A'subloop switch back in Automatic, at which time the TDRFP control valves responded to the zero flow demand by closing. The resulting rapid reduction in flow caused the reactor to l _.

SUMMARY

I LaSalle Unit 1 continued tc coast down during September, operating at the maximum power possible with the exception of the following:

o September 2,19991036 - In preparation for performance of feedwater level control 'B'subloop maintenance, the wrong subloop was taken out of service. The 'A'subloop control switch was mistakenly placed in Manual. The 'A'subloop automatic flow demand signal then decayed to near zero.

When the error was discovered and the 'A'subloop control switch was placed back in Automatic, the corresponding feedwater pump turbine control valve went closed. The resulting rapid reduction in flow caused an automatic reactor scram on low water level.

The resulting forced outage (L1F36) lasted 59.5 hours5.787037e-5 days <br />0.00139 hours <br />8.267196e-6 weeks <br />1.9025e-6 months <br />. The reactor was taken critical at 0414 on September 4,1999, and the generator was placed on line at 2208 on September 4,1999. The unit

, returned to the maximum power possible at approximately 2040 on September 7,1999.

o September 12,1999 0300 - A load drop was taken to support scheduled weekly control rod exercising and surveillance testing. Weekly control rod exercising was delayed due to the loss of position indication on one control rod. Load drop related activities were completed at 1450 on September 12, 1999. The unit then returned to the maximum power possible at approximately 0050 on September 13, 1999. '

o September 19,1999 0000 - A load drop was taken to support withdrawal of control rod 26-35 and weekly control rod exercising. These activities were completed at 0124. The load drop, including the ramp back to maximum possible power lasted 37.5 hours5.787037e-5 days <br />0.00139 hours <br />8.267196e-6 weeks <br />1.9025e-6 months <br />, with the unit returning to maximum available power at approximately 1330 on September 20,1999.

  • September 26,1999 0300 - A load drop was taken to support weekly control rod exercising. Control rod exercising was delayed due to problems with the position indication of one control rod. Control rod exercising was completed at 1729. The ramp back to maximum available power was completed at approximately 0320 on September 27,1999. The entire load drop lasted 24.3 hours3.472222e-5 days <br />8.333333e-4 hours <br />4.960317e-6 weeks <br />1.1415e-6 months <br />.
