Tennessee Valley Authority Sequoyah Nuclear Plant LER SUPPLEMENTAL INFORMATION SQRO-50-327/ 8021 Technical Specification Involved 3.3.3.10 Reported Under Technical Specification 6.9.1.13.c Date of Occurrence: 3/8/80 Identification and Description of Occurrence:

Flow rates for the Auxiliary Building Vent System Sampler Flow Rate Monitor FI-90-101B were not recorded for three shif ts on 3/8/80.

t Conditions Prior to Occurrence:

Unit 1 in Mode 6 following initial core loading. There is currently i no radioactive gaseous releases taking place at Sequoyah.

l Apparent Cause of Occurrence:

The first shif t AUO misread the daily surveillance log and the two  !

subsequent shift AUO's did not question the log entry of "Not Applicable".  !

The log had a column marked " Note" and entries of 1, 2, and 3. The operator misread this and thought it meant " Mode" and that the applicability was Mode 1, 2, and 3. ,

Analysis of Occurrence:

Misread daily surveillance log.

Corrective Action:

The log has been revised to clarify the notation. ,
